Voorgeschiedenis
Ongeveer drie maanden geleden heb ik element 36 endodontisch behandeld. De klachten (doffe pijn, soms scherp bij kauwen) zijn echter niet verdwenen. Op de controle-afspraak van vorige week gaf de patiënt aan dat de pijn nu vaker aanwezig is, vooral ’s avonds.

Onderzoeksresultaten
Koude- en percussietest: licht gevoelig.
Pocketdiepte: normaal.
Röntgenfoto: mogelijk periapicale radiolucentie, maar het beeld is niet helemaal duidelijk.

Ik twijfel over de diagnose en het doel van de verwijzing is:

  • beoordeling door specialist (endodontologie / kaakchirurgie)
  • advies over verdere behandeling (herbehandeling, apicale chirurgie of extractie)
  • eventuele follow-up in jouw praktijk

Ik hoor graag of je deze patiënt kunt overnemen. Indien gewenst stuur ik een kort medisch rapport en de overdrachtsgegevens (röntgenfoto’s en dossier).

History
About three months ago I performed endodontic treatment on tooth 36. The symptoms (dull pain, sometimes sharp when chewing) have not resolved. At the follow-up appointment last week the patient reported that the pain is occurring more frequently, especially in the evenings.

Examination findings
Cold and percussion tests: mildly positive.
Pocket depth: normal.
X-ray: possible periapical radiolucency, but the image is not entirely clear.

I am uncertain about the diagnosis. The purpose of the referral is to:

  • obtain assessment by a specialist (endodontics / oral surgery)
  • receive advice on further treatment (retreatment, apical surgery, or extraction)
  • arrange possible follow-up in your practice

Please let me know if you can accept this patient. If required, I will send a short medical report and the referral documents (X-rays and records).
