Can you look at Mr. López’s image? He says it hurts when chewing on the right side. In the right mandible I see a molar with advanced caries and it looks like there is a small dark area near the tooth root (possible infection or abscess). The maxilla looks normal.
What diagnosis would you give and what plan do you propose: tooth extraction, root canal treatment, or talk already about a prosthesis / implant?

Gracias por el correo. En la radiografía panorámica, en la mandíbula derecha se ve una muela con caries avanzada y una zona radiolúcida cerca de la raíz dental, lo que sugiere una infección periapical posible (absceso).
Yo propondría: 1) Confirmar con exploración clínica y una radiografía periapical pequeña. 2) Si el diente es recuperable, realizar una endodoncia y luego una corona. 3) Si no es recuperable, extracción dental y, más adelante, valorar rehabilitación con implante o prótesis removible según el caso y el presupuesto del paciente.

Hi, Laura:
Thanks for the email. On the panoramic X-ray, in the right mandible you can see a molar with advanced caries and a radiolucent area near the tooth root, which suggests a possible periapical infection (abscess).
I would propose: 1) Confirm with a clinical examination and a small periapical X-ray. 2) If the tooth is salvageable, perform a root canal treatment and then a crown. 3) If it is not salvageable, tooth extraction and, later on, consider rehabilitation with an implant or a removable prosthesis depending on the case and the patient’s budget.
