In the center’s home care service, part of the care is taken on by the family and another part by the volunteer network. The main caregiver is usually a family member, so it is important to clarify responsibilities and negotiate schedules to avoid overload. If a temporary replacement is needed, it is coordinated in advance and the team is informed.

At each visit, the follow-up sheet and the daily report are filled out with incidents, observed medication, and support provided. It is mandatory to respect confidentiality and to have informed consent before sharing data or requesting a referral to social work. (This week I am coordinating my father's home care. I am the main caregiver, but my sister and other relatives from the extended family help us on some days. In addition, a volunteer comes two afternoons a week to keep him company, and that way I can go to work. Yesterday I spoke with her about confidentiality and asked her for informed consent so I could note down the basics. Then I recorded everything on the follow-up sheet and in the daily report. If we see that the responsibility is too much to handle at home, I will request more help and make a referral to the health center.)

Hola, Marta.

Gracias,
Laura

Hi, Marta. I’m Laura, the volunteer from the neighborhood center. I’m coordinating the accompaniment for your aunt Carmen this week.

Your cousin told me that the extended family splits the shifts, but on Wednesday no one can go. Could you confirm who the responsible family member is and which days you need support?

Also, for the in-home assistance, they ask us for a daily log: note the time of arrival, how she is doing, and whether she has taken her medication. Everything is handled with confidentiality. Does that sound okay to you?

Thanks,
Laura

Hola, Laura:

Marta

Hi, Laura:

Thank you for writing. This week the responsible person is me, Marta. My mother can go Monday and Tuesday in the afternoon, and I can go on Thursday after work. Wednesday is the problem: could you accompany her that day around 18:00? If you can’t, I’ll try to ask a neighbor for help.

About the daily log, it seems fine to me. Please note the time of arrival and departure, how she is (mood, pain, whether she has eaten) and whether she has taken her medication. If you notice anything out of the ordinary, let me know on WhatsApp. Thank you for respecting confidentiality.

Best regards,
Marta
