Nota interna: asistencia para ir al baño en cama
En la planta de Medicina Interna se recuerda el protocolo de apoyo a pacientes encamados. Antes de colocar el orinal o la cuña, póngase , proteja la cama con una y ayude al paciente a incorporarse si es posible. Tras la eliminación, realice con jabón neutro o toalla húmeda, seque bien y deseche los residuos según el circuito.
En el registro de enfermería anote la hora y describa el aspecto: (clara, amarilla u oscura), (líquida, pastosa o sólida) y (escasa, moderada o abundante). Si hay escape urinario o fecal, cambie el y las sábanas si están húmedas. Informe al equipo si aparecen olor fuerte, sangre o ausencia de diuresis.In the Internal Medicine ward, the support protocol for bedridden patients is being reminded. Before placing the urinal or the bedpan, put on disposable gloves, protect the bed with a waterproof cover, and help the patient sit up if possible. After elimination, carry out intimate hygiene with neutral soap or a wet wipe, dry well, and dispose of the waste according to the procedure.
In the nursing record, note the time and describe the appearance: coloration (clear, yellow or dark), consistency (liquid, pasty or solid) and amount (scant, moderate or abundant). If there is urinary or fecal leakage, change the adult diaper and the sheets if they are wet. Inform the team if a strong odor, blood, or absence of urination appears.

Si el paciente pide ir al baño, le ayudaré a incorporarse y le colocaré el orinal o el recipiente para heces. Observaré y registraré la hora y el aspecto: color, consistencia (si procede) y cantidad. Si aparece dolor, olor fuerte, sangre o un cambio claro en la coloración o la cantidad, avisaré a enfermería.
Hi Marta, thanks for the handover. On my shift I will check room 12 at the beginning and make sure the bed is dry. If there is new urinary leakage, I will perform intimate hygiene, change the diaper and the waterproof protector, and dry the area well.
If the patient asks to go to the bathroom, I will help him sit up and place the urinal or the bedpan for stools. I will observe and record the time and appearance: color, consistency (if applicable), and amount. If pain appears, a strong odor, blood, or a clear change in color or amount, I will inform nursing.
