In the internal medicine ward, beta-blockers are medications often used for patients with hypertension or arrhythmias. During hospitalization, staff must also observe signs of possible malnutrition, such as unintentional weight loss, reduced appetite, and decreased muscle mass, and propose dietary support if the patient is unable to follow the prescribed diet.

ho controllato i segni vitali del Sig. Bianchi: PA 145/85, FC 78, T 36,6. In cartella non ci sono controindicazioni note al beta-bloccante; lo monitorerò per possibili effetti collaterali come capogiri o bradicardia. Dopo la prima somministrazione ricontrollo PA e FC e ti aggiorno.

Per la vitamina IM: dato che il paziente è magro e ha perdita di peso involontaria e riduzione dell'appetito, valuto prima la massa muscolare. Per ridurre il rischio di ematoma preferisco il ventrogluteo rispetto al deltoide, perché è più protetto e sicuro nei pazienti con poca massa al braccio. Se sei d'accordo, preparo anche una nota per il medico con proposta di supporto dietetico.

Hi Laura,

I checked Mr. Bianchi’s vital signs: BP 145/85, HR 78, T 36.6. In the chart there are no known contraindications to the beta-blocker; I will monitor him for possible side effects such as dizziness or bradycardia. After the first administration I will recheck BP and HR and update you.

For the IM vitamin: since the patient is thin and has unintentional weight loss and reduced appetite, I will first assess muscle mass. To reduce the risk of hematoma I prefer the ventrogluteal site over the deltoid, because it is more protected and safer in patients with little arm muscle mass. If you agree, I will also prepare a note for the doctor with a proposal for dietary support.
