In reparto, nelle ultime settimane sono aumentate le segnalazioni di pazienti con sintomi di . I segnali più comuni sono , , forte confusione e insonnia. In questi casi l'operatore mantiene la calma, parla con frasi brevi e verifica se la persona sta assumendo la terapia prescritta.
Se compaiono (agitazione, paura intensa, frasi incoerenti), si valuta il per sé e per gli altri e si avvisa subito il medico di guardia. Per i pazienti con o disturbi del neurosviluppo, come la sindrome di Rett, si propone un supporto psicosociale e si coinvolge la famiglia per raccogliere informazioni utili.On the ward, in recent weeks there has been an increase in reports of patients with symptoms of psychosis. The most common signs are hallucinations, delusions, severe confusion and insomnia. In these cases the staff member stays calm, speaks in short sentences and checks whether the person is taking the prescribed therapy.
If crisis signs appear (agitation, intense fear, incoherent sentences), the risk to themselves and to others is assessed and the on-call doctor is alerted immediately. For patients with intellectual disability or neurodevelopmental disorders, such as Rett syndrome, psychosocial support is offered and the family is involved to gather useful information.

Psicologa: Show Senti, da stamattina Marco è molto agitato: ha schizofrenia, parla da solo e dice di avere allucinazioni visive e uditive molto forti.

Per favore, nel tuo turno del pomeriggio, prova a monitorare il suo stato, rassicurarlo e valutare se è necessario chiamare lo psichiatra per una valutazione psichiatrica urgente.

Hi Marco,
today during the morning shift Mr. Rinaldi (35 years old), being treated for schizophrenia, was very agitated. He said he was hearing voices insulting him and he had the feeling that the other patients were talking about him. It seems to me a possible worsening psychosis, with auditory hallucinations and perhaps persecutory delusions.
Please, during your afternoon shift, try to monitor his condition, reassure him and assess whether it is necessary to call the psychiatrist for an urgent psychiatric assessment.
Will you write to me at the end of the shift with how it went and what you propose to manage the situation?

le scrivo per informarla su come è andato il turno con il signor Rinaldi. All’inizio del pomeriggio era ancora un po’ agitato e riferiva <strong>allucinazioni</strong> uditive: diceva che le voci lo criticavano e che gli altri lo guardavano in modo strano. Ho parlato con lui in un luogo tranquillo, l’ho rassicurato e ho spiegato che qui è in un posto sicuro.
Ho continuato a <strong>monitorare</strong> il suo stato ogni 30 minuti. Dopo la terapia di gruppo è sembrato più calmo, ma la <strong>perdita di contatto con la realtà</strong> non è scomparsa del tutto.
Secondo me sarebbe utile una <strong>valutazione psichiatrica</strong> nelle prossime ore per valutare una possibile modifica della terapia e un piano per <strong>gestire</strong> meglio i prossimi <strong>segnali di crisi</strong>.

Dear Laura,
I am writing to inform you about how the shift with Mr. Rinaldi went. At the beginning of the afternoon he was still a bit agitated and reported auditory <strong>hallucinations</strong>: he said that the voices were criticizing him and that the others were looking at him in a strange way. I spoke with him in a quiet place, reassured him, and explained that he is in a safe place here.
I continued to <strong>monitor</strong> his condition every 30 minutes. After group therapy he seemed calmer, but the <strong>loss of contact with reality</strong> did not disappear completely.
In my opinion a <strong>psychiatric assessment</strong> would be useful in the next few hours to evaluate a possible change in therapy and a plan to <strong>manage</strong> future <strong>warning signs of a crisis</strong> better.
