On the ward, for patients with dysphagia it is mandatory to follow the modified-texture diet indicated by the dietitian. Liquids must be thickened when prescribed and posture during the meal must be monitored to reduce the risk of aspiration. Offer small sips, use aids (spoon or adapted cup) and stop administration if coughing or a “gurgly” voice appears.

At the end of the shift fill in the I/O chart: record how much the patient drank and how much they urinated. Immediately report to the team any signs of dehydration (dry mucous membranes, tiredness, dark urine) or if the patient refuses liquids. In the case of enteral nutrition, check that the tube is secured and that the prescription is followed.

Stamattina ho assistito la signora Riva durante la colazione in reparto. Ha la disfagia e ieri aveva tossito bevendo, quindi oggi ho usato l'addensante e le ho fatto fare piccoli sorsi, mantenendo la postura corretta. La dieta è modificata, tutta passata. Ho controllato che non ci fossero segni di aspirazione e ho annotato nel diario dell'assunzione quanto ha bevuto e quanta urina ha eliminato per il bilancio idrico. A fine turno ho notato secchezza delle mucose e un po' di pallore, e ho segnalato tutto al team sanitario.

ti confermo che questa mattina il Sig. Rinaldi ha tossito dopo alcuni sorsi d'acqua e in generale ha bevuto poco. Ho applicato la profilassi dell'aspirazione: liquidi addensati e piccoli sorsi, assistendolo e mantenendo la postura corretta durante i pasti.

Per il bilancio idrico: ho registrato 350 ml di liquidi assunti e aggiornerò le uscite nel registro entro la fine del turno. A pranzo ha consumato circa metà della dieta modificata (passata) e ha preso solo piccoli sorsi di liquido.

Ho notato la mucosa un po' secca, ma non altri segni evidenti di disidratazione. Se ci sono cambiamenti, ti avviso subito.

I confirm that this morning Mr. Rinaldi coughed after a few sips of water and in general drank very little. I applied aspiration prophylaxis: thickened liquids and small sips, assisting him and maintaining correct posture during meals.

For the fluid balance: I recorded 350 ml of fluids taken and I will update the outputs in the chart by the end of the shift. At lunch he ate about half of the modified diet (puréed) and took only small sips of liquid.

I noticed the mucosa was a bit dry, but no other obvious signs of dehydration. If there are any changes, I will let you know right away.
