Nella U.O. di Medicina Interna, da questa settimana la valutazione del dolore viene registrata in cartella a ogni turno. Si usa la Scala di Valutazione Numerica ( ) e, se il paziente non collabora, una scala comportamentale: si osservano postura, mimica, agitazione e respiro come non verbali. In caso di variazioni improvvise, va anche l'effetto della terapia e il livello di funzione.

Nel briefing è ricordato che la documentazione deve essere coerente con la classificazione e con i bisogni funzionali di ; quando possibile si effettua una valutazione multidimensionale secondo il modello . Per chiamare il medico si usa : situazione, background, valutazione, raccomandazione e risposta attesa, così le informazioni rilevanti sono chiare e complete.
In the Internal Medicine Unit, starting this week pain assessment is recorded in the chart at every shift. The Numerical Rating Scale (NRS) is used and, if the patient does not cooperate, a behavioral scale: posture, facial expression, agitation and breathing are observed as nonverbal indicators. In case of sudden changes, the effect of therapy and the level of function must also be reported.

In the briefing it is reminded that documentation must be consistent with the WHO classification and with Gordon’s functional health patterns; when possible, a multidimensional assessment is carried out according to the Positive Health model (six domains). To call the physician, SBARR is used: situation, background, assessment, recommendation and expected response, so relevant information is clear and complete.

Stamattina, durante la routine di assistenza, ho rivalutato il dolore del signor Rinaldi e ho aggiornato la documentazione clinica. Alla Scala di Valutazione Numerica, la NRS, ha detto 7 su 10, ma parlava poco. Per questo ho osservato anche gli indicatori non verbali del dolore e ho usato una scala comportamentale: era teso e proteggeva l'addome.
