Notice for the shift: during personal hygiene, check the skin of bedridden patients. Observe especially heels, sacrum, and elbows: if the skin becomes red or is warm, report it immediately and change position. If there is a wound or a burn, clean it and apply a simple dressing according to procedure.

Record in the chart: area, color, pain, and presence of discharge. If the skin is swollen, bleeding, or there is a risk of infection, notify the responsible nurse. Also assess pressure points and use a bandage only if indicated.
