The symptoms may fit psychosis or schizophrenia, but on duty the most important things are safety and a quick assessment of the risk of self-harm.
In contact with the patient, it is recommended to speak calmly, use short sentences, and not challenge subjective experiences. It is worth asking what the patient hears or sees and whether they feel threatened. In the case of people with an intellectual disability or neurodevelopmental disorders (e.g., Rett syndrome), communication should be adapted: more time, simple choices, and the support of a close person. A deterioration in condition is reported to the doctor on duty and a crisis intervention is initiated.

I’m in the emergency department and I need your help. We have a patient (around 30 years old) who says he hears voices and sees shadows — they look like hallucinations. He is very agitated, has delusions (claims that someone is following him), and finds it hard to distinguish reality from what he is experiencing. There was also a brief panic attack.
