Jeśli pacjent pyta o plan leczenia, udzielamy informacji w sposób zrozumiały i sprawdzamy, czy wyraża . W sytuacji konsultujemy decyzję z lekarzem dyżurnym lub pielęgniarką oddziałową. Każde naruszenie oraz zdarzenie niepożądane należy zgłosić zgodnie z procedurą, aby poprawiać .In connection with the recent report, we would like to remind you of the rules for working on the ward: we protect the patient’s privacy and we do not discuss their health condition in the elevator or in the corridor. We update the medical documentation on an ongoing basis and we do not leave it in places accessible to unauthorized persons.
If a patient asks about the treatment plan, we provide information in a clear way and check whether they give informed consent. In the event of an ethical conflict, we consult the decision with the on-call doctor or the ward nurse. Any breach of data confidentiality and any adverse event must be reported according to the procedure in order to improve the quality of care.

wczoraj po południu na dyżurce znalazłam wydruk z danymi pacjenta (imię, PESEL, rozpoznanie). To może być naruszenie poufności danych i praw pacjenta. Proszę o krótką informację, czy Pani drukowała te dokumenty i co się stało. Jeżeli to błąd, musimy zrobić zawiadomienie o błędzie zgodnie z procedurą i ocenić ryzyko. Proszę też sprawdzić, czy w dokumentacji medycznej wszystko jest kompletne.

yesterday afternoon on duty I found a printout with a patient’s data (first name, PESEL number, diagnosis). This may be a breach of data confidentiality and patients’ rights. Please provide a brief note on whether you printed these documents and what happened. If this was a mistake, we need to file an incident report in accordance with the procedure and assess the risk. Please also check whether everything is complete in the medical documentation.

Dziękuję za wiadomość. Wczoraj drukowałam wyniki tego pacjenta do dokumentacji medycznej. Najprawdopodobniej zostawiłam wydruk na dyżurce, gdy musiałam pilnie wejść na salę pacjenta. Po dyżurze nie zauważyłam brakującej kartki.
Rozumiem, że to może być naruszenie poufności danych. Proponuję, żebym jeszcze dziś rano opisała sytuację i złożyła zawiadomienie o błędzie zgodnie z procedurą. Mogę też sprawdzić, czy wydruk nie trafił do niewłaściwych dokumentów i potwierdzić kompletność dokumentacji pacjenta.
Na przyszłość będę odkładać wydruki od razu do zamkniętej teczki, aby chronić dane pacjenta.

Good morning Ms Magdalena,
Thank you for your message. Yesterday I printed this patient’s results for the medical documentation. Most likely I left the printout at the nurses’ station when I had to urgently go into the patient’s room. After my shift I did not notice the missing sheet.
I understand that this may be a breach of data confidentiality. I suggest that this morning I describe the situation and file an incident report in accordance with the procedure. I can also check whether the printout did not end up in the wrong documents and confirm the completeness of the patient’s documentation.
In the future I will put printouts immediately into a closed folder in order to protect the patient’s data.
