Wczoraj odbyła się pierwsza wizyta pielęgniarki środowiskowej u pana Jana, który mieszka z żoną i dorosłą córką. Córka jest główną , ale pracuje na pełen etat, dlatego rodzina poprosiła o wsparcie z lokalnego stowarzyszenia. Wolontariuszka ma pomagać w czynnościach, na przykład przy przygotowaniu lekkiego posiłku, krótkim spacerze i podstawowych parametrów zdrowia.
W domowej pielęgniarka zapisała opieki, w tym podział zadań między rodzinę, wolontariuszkę i zespół medyczny. Zaznaczyła, że rodzina chce zadbać o pacjenta i potrzebuje także emocjonalnego. Ustalono, że w razie objawów córka ma natychmiast zadzwonić do przychodni i wizytę lekarza rodzinnego. Na koniec wizyty pielęgniarka sporządziła krótką notatkę pielęgniarską do przekazania na dyżur koleżance z zespołu.Yesterday the first visit of the community nurse took place at Mr. Jan’s home; he lives with his wife and adult daughter. The daughter is the main caregiver, but she works full-time, so the family asked for support from a volunteer from a local association. The volunteer is to help with daily activities, for example preparing a light meal, taking a short walk, and monitoring basic health parameters.
In the home documentation, the nurse wrote down the care plan, including a division of tasks among the family, the volunteer, and the medical team. She noted that the family wants to ensure the patient’s privacy and also needs emotional support. It was agreed that in the event of worrying symptoms, the daughter is to immediately call the clinic and arrange a visit from the family doctor. At the end of the visit, the nurse prepared a short nursing note to hand over to the colleague on duty from the team.

Hi Marta,
I have a big favor to ask you. On Saturday from 10:00 to 14:00 I have to go with my son to a sports competition. Usually at that time I’m at Grandma’s and help her with everyday activities – I do the shopping, give her her medicines, monitor her health condition.
Could you take over the care for those few hours on Saturday? Grandma is independent, but you need to ensure her safety and write a short note in our home documentation.
Of course I will pay you for your time.

Hi Anna,
thank you for your message and for your trust. On Saturday I have a dentist appointment until 11:30, so I can’t be at your grandmother’s from 10:00. However, I could come from around 12:00 to 14:00.
If that helps, I can do the shopping, remind her about her medicines and write a short note in the home documentation: how Grandma was feeling and whether she reported any ailments. If you need someone already from 10:00, I can ask a colleague whether she has time to cover earlier.
