Notatka służbowa: leki, zastrzyki i ryzyko niedożywienia
In the ward, we remind you: when admitting a patient, check whether they use beta-blockers and when they took the last dose. Pay attention to possible side effects (e.g. dizziness, a drop in blood pressure ) and whether the patient is not taking an anticoagulant at the same time — then the risk of bleeding after an injection increases.
For an intramuscular injection , the choice of site depends on age and muscle mass. In adults, the deltoid muscle or the ventrogluteal site is most often chosen; in children and very thin people, the vastus lateralis muscle of the thigh is safer. Also assess signs of malnutrition : weight loss, muscle weakness, difficulty eating. If the patient has a problem with following the diet , offer simple recommendations and a referral to a dietitian.

Wizyta kontrolna - beta-bloker
(Good morning, I see in your records that you are taking a beta blocker for hypertension—how have you been feeling over the last two weeks?)
(Overall better, my blood pressure has gone down, but sometimes I feel dizzy and it seems to me that my pulse is lower than usual.)
Lekarka: Show To może zdarzać się przy beta-blokerach - trzeba uważać na zbyt wolne tętno i spadki ciśnienia; proszę mierzyć ciśnienie i tętno rano i wieczorem i zapisywać wyniki.
(That can happen with beta blockers—you need to watch out for a pulse that is too slow and drops in blood pressure; please measure your blood pressure and pulse in the morning and in the evening and write down the results.)
(Additionally, I’m taking an anticoagulant after the procedure—does that conflict with the beta blocker?)
(Usually there is no significant interaction, but please watch for bleeding and do not stop the medications on your own; at the next visit we will assess whether to change the beta-blocker dose.)
Open questions:
1. Dlaczego lekarka prosi pacjenta o prowadzenie dzienniczka ciśnienia i tętna przed zmianą dawki?
Why does the doctor ask the patient to keep a blood pressure and pulse diary before changing the dose?

Dodatkowo: jakie 2–3 proste kroki możemy zrobić, żeby wesprzeć jedzenie i picie?
Pozdrawiam,
Anna Nowak, piel. koordynująca
Good morning,
I would like to ask for a brief confirmation of today’s plan for Mr Król (78 years old). The doctor has ordered an analgesic intramuscularly. The patient has low muscle mass and a poor appetite; for a week he has been eating very little. Medical history: hypertension; he takes a beta-blocker and an anticoagulant.
Could you write which injection site you choose (e.g. deltoid muscle or ventrogluteal) and what to pay attention to during observation after administration? Additionally: what 2–3 simple steps can we take to support eating and drinking?
Kind regards,
Anna Nowak, coordinating nurse

(Regarding eating, we can start by…)
Pozdrawiam,

Good morning Ms Anna,
I suggest administering the medication intramuscularly in the ventrogluteal area. Mr Król has low muscle mass, so this site is safer than the deltoid muscle, where absorption may be lower and pain greater.
After administration, please check blood pressure and observe general well-being (dizziness, weakness), because the patient is taking a beta-blocker. Due to the anticoagulant, let’s also watch for bleeding and a possible hematoma at the injection site.
Regarding eating and drinking, I suggest: 1) small, more frequent portions (every 2–3 hours), 2) ask the patient what he likes and adjust meals accordingly, 3) keep water within reach and remind him to drink. If the amount of food does not improve, I will inform the doctor and the dietitian.
Kind regards,
