W naszym szpitalu każda pielęgniarka ocenia, czy pacjent może samodzielnie skorzystać z toalety, czy potrzebuje lub . Przed podaniem naczynia należy zabezpieczyć łóżko i w bezpiecznej pozycji, na przykład na boku lub w półsiedzącej. Po lub stolca personel powinien natychmiast opróżnić i sprawdzić kolor oraz , a także i ewentualny zapach.
Jeśli pacjent ma lub stolca, ważna jest regularna zmiana pieluchy, aby skóra nie była długo wilgotna. Po każdym zabrudzeniu należy dokładnie umyć okolice intymne letnią wodą z delikatnym środkiem myjącym, a potem starannie . W dokumentacji pielęgniarskiej trzeba krótko opisać sytuację i , na przykład ciemne zabarwienie moczu, bardzo rzadki stolec lub przepełnioną pieluchę.In our hospital, every nurse assesses whether the patient can use the toilet independently or needs a bedpan or a urinal. Before providing the container, the bed should be protected with a protective pad and the patient should be positioned in a safe position, for example on their side or in a semi-sitting position. After passing urine or stool, staff should immediately empty the container and check the color and volume of the urine, as well as the consistency of the stool and any possible odor.
If the patient has urinary or fecal incontinence, regular diaper changes are important so that the skin is not damp for a long time. After each soiling, the intimate area should be washed thoroughly with lukewarm water and a mild cleansing agent, and then the skin should be carefully dried. In the nursing documentation, the situation should be briefly described and any abnormality should be reported, for example dark-colored urine, very loose stool, or an overfilled diaper.

Mocz miał jasnożółte zabarwienie, objętość była średnia. Stolec był miękki, o normalnym zapachu. Nie zauważyłam krwi ani innych niepokojących zmian. Będę obserwować pacjentkę i w razie potrzeby zgłoszę nieprawidłowość w dokumentacji.

Good morning, Ms. Anna,
I am writing to inform you about the help provided to Mrs. Kowalska in the afternoon. The patient could not get to the bathroom, so I helped her use a bedpan in bed. First I helped her sit down, and after she passed urine and stool I washed the intimate area and thoroughly dried the skin.
The urine was light yellow in color, the volume was medium. The stool was soft, with a normal smell. I did not notice blood or any other worrying changes. I will continue to observe the patient and, if necessary, I will report an irregularity in the documentation.
