Notatka służbowa: ocena bólu i przekaz SBAR
Notatka wewnętrzna oddziału: przy każdym przyjęciu i po zmianie leczenia oceniamy ból pacjenta. Prosimy stosować (0–10) lub, gdy pacjent ma trudności z mówieniem, obserwować : grymas twarzy, napięcie mięśni, unikanie ruchu. Wynik wpisujemy do i krótko opisujemy funkcjonowanie pacjenta według wzorców Gordona.
Podczas przekazywania dyżuru stosujemy : sytuacja, tło, ocena, rekomendacja. W ocenie uwzględniamy także podejście „ ” (Huber) — nie tylko chorobę, lecz także zasoby i cele pacjenta. Jeśli ból się nasila lub pojawiają się nowe ograniczenia, i zaproponuj dalsze działania zgodne z .Internal ward memo: at every admission and after a change in treatment, we assess the patient’s pain. Please use the NRS (0–10) or, if the patient has difficulty speaking, observe non-verbal signs : facial grimace, muscle tension, avoiding movement. We record the score in the care plan and briefly describe the patient’s functioning according to Gordon’s patterns.
During shift handover we use SBAR : situation, background, assessment, recommendation. In the assessment we also take into account the “ Positive Health ” approach (Huber) — not only the illness, but also the patient’s resources and goals. If the pain intensifies or new limitations appear, report a change in condition and propose further actions in line with the WHO classification .

I’m new on the ward and I’m taking over room 12. Could you please pass on information about Mr Nowak (post-op)? He says it hurts a lot, but it’s hard for me to interpret. What is his pain rating on the NRS (0–10) and have you noticed any non-verbal symptoms (e.g., grimacing, tensing up, avoiding movement)? What was given and do we have anything to monitor overnight? If you can, write briefly in SBAR format.

I’m passing on the information in SBAR format.
S (situation): Mr Nowak, room 12, post orthopedic procedure, reports pain around the wound.
B (background): Operated on this morning; after mobilization the pain increased.
A (assessment): On the NRS he rated the pain as 7/10 with movement and 4/10 at rest. Non-verbally, grimacing and tensing were visible; he avoided changing position. Vital signs stable.
R (recommendation): Monitor NRS every 4 hours and after administering medication; observe pain behavior when getting up. If the pain increases above 8/10 or sudden symptoms appear (e.g., shortness of breath, drop in blood pressure), report to the on-call doctor.
