In urgent cases, it is best to start with the family doctor, who assesses the state of health and, if necessary, issues a referral to a specialist clinic or to the hospital. Many matters can be handled through outpatient care or during home visits by a community nurse.
When planning treatment, it is worth checking what health insurance covers. Some services are subject to limits, and for medications it is reimbursement and the patient’s co-payment that matter. When longer inpatient care is needed, the decision depends on medical indications and the availability of places in long-term care facilities or in social welfare homes.

Wypis ze szpitala też posiadam, ale dziś nie mam dostępu do skanera. Mogę wysłać zdjęcia dokumentów wieczorem albo przynieść je osobiście. Czy w tej sytuacji możliwe są wizyty pielęgniarki środowiskowej w domu i od kiedy byłyby dostępne? Proszę też o informację, jakie dokumenty są potrzebne do przygotowania planu opieki.
