Also measure pulse and breathing and record the results at the same time.
If you notice a change in level of consciousness, shortness of breath, or cold sweat, report it to the nurse on duty immediately. In the description of the condition, state exactly what you see and when it happened. Do not add guesses.

Good morning, Marta. I have already measured Mr Nowak’s parameters: temperature 38.1°C, pulse 110/min, blood pressure 95/60 mmHg. His skin is pale and slightly moist. Breathing is rapid, the patient is drowsy, speaks slowly, but answers questions. I am reporting these as worrying observations. Now I will give him water, make him comfortable, and in 5 minutes I will measure his blood pressure again. If it gets worse, I will call immediately.
