Notatka służbowa: obserwacja podczas posiłków i bilans płynów

W związku z kilkoma incydentami krztuszenia podczas kolacji prosimy personel o szczególną obserwację osób z podejrzeniem . Zwracaj uwagę na kaszel po przełknięciu, mokry głos, zaleganie jedzenia w jamie ustnej oraz . Jeśli konieczna jest zmiana konsystencji, stosuj dietę o zmodyfikowanej teksturze lub napoje zgodnie z zaleceniem. Przy karmieniu używaj pomocniczych sztućców i przypominaj o małych kęsach.

Od dziś obowiązuje dokładny (przyjmowanie/oddawanie). Notuj wszystkie oraz ilość oddanego moczu; przy wątpliwościach częściej. Zgłaszaj objawy : senność, zawroty głowy, mała ilość moczu lub ciemny mocz. Gdy pacjent nie pije wystarczająco, uzupełniaj płyny zgodnie z planem i informuj pielęgniarkę dyżurną.
Due to several choking incidents during dinner, we ask staff to pay special attention to people with suspected swallowing disorders. Watch for coughing after swallowing, a wet voice, food remaining in the mouth, and dry mouth. If a change in consistency is necessary, use a modified-texture diet or thicken drinks as recommended. When feeding, use assistive cutlery and remind them to take small bites.

From today, a detailed fluid balance (intake/output) is in effect. Record all oral fluids and the amount of urine passed; if in doubt, monitor diuresis more frequently. Report signs of dehydration: drowsiness, dizziness, low urine output, or dark urine. If the patient is not drinking enough, replenish fluids according to the plan and inform the nurse on duty.
