Notatka z przekazania dyżuru (oddział internistyczny): Pacjent po z dusznością. Obecnie mniejsze, bez bólu w klatce piersiowej. stabilne: temperatura ciała prawidłowa, w normie, tętno równe. Monitorować stan co 4 godziny i zanotować obserwacje w . lekarza: zebrać wywiad o alergiach i lekach z domu. W razie zmiany stanu zgłaszać dyżurnemu. Przy wypisie wypisać kartę informacyjną i sporządzić krótki raport dla poradni.
Shift handover note (internal medicine ward): Patient admitted with shortness of breath. Currently symptoms are milder, no chest pain. Vital signs stable: body temperature normal, blood pressure within normal range, pulse regular. Monitor condition every 4 hours and record observations in the patient chart.

Doctor’s order: take a history of allergies and home medications. In case of any change in condition, report to the on-call physician. At discharge, complete the discharge summary and prepare a brief report for the outpatient clinic.

Stan pacjenta: stabilny, przytomny, w kontakcie logicznym. Chodzi z pomocą jednej osoby.

Parametry życiowe o 8:00:

  • Ciśnienie krwi: 135/80 mmHg
  • Tętno: 78 / min
  • Temperatura ciała: 37,2°C
  • Saturacja: 96% na powietrzu

Ból: pacjent zgłasza ból w nodze (4/10), ból większy przy chodzeniu. Pozycja siedząca – ból mniejszy.

Zabiegi pielęgnacyjne: pomoc w higienie porannej, zmiana opatrunku na prawej nodze (rana czysta, sucha). Monitorowanie rany – bez zmian.

Podawanie leków: leki doustne zgodnie ze zleceniem lekarskim, bez problemów z połykaniem. Lek przeciwbólowy o 9:00.

Ważne informacje (uwagi):

  • Proszę wykonać pomiar parametrów życiowych jeszcze raz o 20:00.

Vital signs at 8:00:

Pain: the patient reports pain in the leg (4/10), pain is worse when walking. Sitting position – less pain.

Na mojej zmianie (19:00–7:00) planuję wykonać pomiar parametrów życiowych o 20:00. Będę też monitorować ból nogi i ranę.

Mam jedno pytanie: czy pacjent ma jakieś problemy z chodzeniem do toalety w nocy? Czy potrzebuje pomocy jednej osoby, czy raczej wózka?

Jeśli zauważę zmianę stanu pacjenta (większy ból, gorączka, duszność), zgłoszę to lekarzowi dyżurnemu.
