Herr Weber ist 45 Jahre alt und arbeitet als Ingenieur in Deutschland. Heute kommt er zu einer neuen Zahnärztin, Frau Dr. König. Er spricht gut Englisch, aber sein Deutsch ist noch nicht perfekt.
Bei der Untersuchung sieht die Zahnärztin: Ein Backenzahn ist stark zerstört, eine alte Füllung ist gebrochen und ein Zahn fehlt schon seit zwei Jahren. Sie erklärt langsam und deutlich: „Für den zerstörten Zahn empfehle ich eine . Mit einer schützen wir den Zahn und verbessern die . Für die Lücke können wir eine machen. Die ist fest, sie sitzt auf den Nachbarzähnen.“
Sie zeigt Herrn Weber ein Modell: „Hier sehen Sie eine Vollkeramik-Krone aus . Sie ist sehr ästhetisch, besonders im Frontzahnbereich. Eine -Krone ist auch stabil, aber man sieht manchmal einen dunklen Rand. Eine Krone nur aus ist sehr stabil bei hoher Belastung, zum Beispiel im Seitenzahnbereich, aber sie ist nicht so schön.“
Dann erklärt sie den Ablauf: „Zuerst machen wir die : Wir formen den Zahn. Dann machen wir einen Abdruck. Sie bekommen ein provisorisches Teil, ein Provisorium, bis das Labor fertig ist. Am Ende kontrollieren wir die Okklusion und zementieren die Krone oder die Brücke fest.“
Herr Weber stellt viele Fragen. Er möchte ein gutes funktionelles Ergebnis und eine gute Ästhetik. Am Ende entscheidet er sich im Frontbereich für Zirkon und im Seitenbereich für Metallkeramik. Er macht einen neuen Termin für die Präparation und den Abdruck.Mr. Weber is 45 years old and works as an engineer in Germany. Today he is visiting a new dentist, Dr. König. He speaks good English, but his German is not yet perfect.
During the examination the dentist notices: a molar is severely damaged, an old filling is broken, and one tooth has been missing for two years. She explains slowly and clearly: "For the damaged tooth I recommend a crown. A crown protects the tooth and improves its appearance. For the gap we can make a bridge. The bridge is fixed — it sits on the neighboring teeth."
She shows Mr. Weber a model: "Here you can see a full-ceramic crown made of zirconia. It is very aesthetic, especially for front teeth. A metal-ceramic crown is also stable, but sometimes you can see a dark margin. A crown made only of metal is very strong under heavy load, for example in the back of the mouth, but it is not as attractive."
Then she explains the procedure: "First we prepare the tooth: we shape it. Then we take an impression. You will get a temporary restoration, a provisional, until the lab finishes the final piece. At the end we check the bite and cement the crown or the bridge permanently."
Mr. Weber asks many questions. He wants a good functional result and pleasing aesthetics. In the end he chooses zirconia for the front teeth and metal-ceramic for the back teeth. He makes a new appointment for the preparation and the impression.

Ich entscheide mich für eine Metall-Keramik-Krone. Der Zahn steht im Seitenzahnbereich und hat eine starke Belastung, deshalb möchte ich ein stabiles Material. Metall-Keramik hat eine gute Stabilität und auch eine gute Ästhetik. Vollmetall wäre funktionell gut, aber der Patient wünscht eine bessere Ästhetik. Zirkoniumoxid ist auch ästhetisch, aber bei starkem Knirschen bevorzuge ich hier Metall-Keramik.
Für die Zementation plane ich einen konventionellen Glasionomer-Zement zu verwenden. Die Präparation hat eine gute Retention, deshalb ist das möglich.
Ein einfaches Kunststoff-Provisorium ist für die Zwischenzeit in Ordnung.

Good day Ms. Keller,
thank you for your email and for noting the high occlusal load on tooth 26.
I recommend a metal-ceramic crown. The tooth is in the posterior region and is subject to heavy forces, so I prefer a stable material. Metal-ceramic provides good strength while offering acceptable aesthetics. Full metal would be functionally strong, but the patient prefers improved appearance. Zirconia is also aesthetic, but given the pronounced bruxism I favor metal-ceramic in this case.
For cementation I plan to use a conventional glass ionomer cement. The preparation has good retention, so this is suitable.
A simple resin temporary restoration is acceptable for the interim.
