Dr. Weber ist Zahnarzt in Köln. Heute hat er einen neuen Patienten, Herrn Silva. Herr Silva ist Ingenieur aus Brasilien und arbeitet seit zwei Jahren in Deutschland. Er spricht schon ganz gut Deutsch.
Herr Silva hat seit einigen Tagen starke Zahnschmerzen. Der Zahn ist entzündet und sehr empfindlich bei kalten und heißen Getränken. Dr. Weber erklärt ruhig: „Wir machen eine . Zuerst bekommen Sie eine . Dann ist der Zahn bald und Sie haben keine .“
Nach der öffnet Dr. Weber den Zahn und reinigt die Wurzelkanäle mit feinen Wurzelkanalinstrumenten und Feilen. Danach füllt er die Kanäle mit einem und Füllmaterial. Zum Schluss macht er eine Röntgenaufnahme, um das Ergebnis zu kontrollieren.
Dr. Weber spricht auch über mögliche : „Manchmal können die Schmerzen wieder auftreten. Der Zahn kann ein paar Tage überempfindlich sein. Wenn die Schmerzen sehr stark sind oder wenn es blutet, rufen Sie bitte sofort in der Praxis an.“
Zum Ende bekommt Herr Silva Nachsorgehinweise: Er soll vorsichtig essen und heute auf Wärme verzichten. Außerdem macht Dr. Weber einen Folgetermin in zwei Wochen. I was at your practice yesterday for a root canal treatment on tooth 26. The anesthesia worked well and I had no pain during the procedure.
Since this morning I have had severe pain when chewing and a feeling of pressure. I am already taking the painkiller you recommended. It helps somewhat, but not completely.
You told me there was an apical periodontitis on the tooth and that pain can occur after the treatment. Nevertheless, I am a bit worried about complications. Could you please explain the risk again in simple words? When is it normal and when is it dangerous?
I would also like to ask:
- Should I start taking the antibiotic now, or wait a little longer?
- Do I need to rinse the gum additionally or apply something cold?
- Will the drainage remain in the tooth until the next appointment?
The follow-up appointment is next week at 10 a.m. If the pain increases, should I come to the practice immediately, or is next week’s appointment sufficient?

Leichte bis mittlere Schmerzen nach einer Wurzelkanalbehandlung sind in den ersten Tagen normal. Der Knochen an der Wurzelspitze war entzündet (apikale Parodontitis). Durch die Behandlung ist das Gewebe gereizt. Deshalb haben Sie jetzt Druckgefühl beim Kauen.
Bitte nehmen Sie das Schmerzmittel, wie wir in der Praxis besprochen haben. Wenn das Schmerzmittel gut hilft und Sie kein Fieber haben und keine starke Schwellung sehen, ist das meistens nicht gefährlich.
Das Risiko für Komplikationen ist höher, wenn:
- die Schmerzen sehr stark sind und nicht besser werden,
- Sie Fieber bekommen,
- die Wange stark anschwillt,
- Sie den Mund kaum öffnen können.
In diesen Fällen kommen Sie bitte sofort ohne Termin in die Praxis oder rufen Sie den zahnärztlichen Notdienst an.
Das Antibiotikum nehmen Sie bitte <strong>jetzt</strong> ein, wie im Rezept steht: 3-mal täglich, 5 Tage lang, nach dem Essen. Kühlen Sie die Wange von außen leicht mit einem Kühlpack (nicht direkt auf die Haut) und putzen Sie die Zähne normal weiter. Das Zahnfleisch müssen Sie nicht extra spülen, eine normale Mundhygiene ist genug.
Die Drainage bleibt bis zur nächsten Sitzung im Zahn. Bei der Nachkontrolle nächste Woche mache ich ein neues Röntgenbild und kontrolliere die Situation. Wenn die Schmerzen gleich bleiben oder langsam weniger werden, reicht der Termin nächste Woche. Wenn die Schmerzen heute oder morgen deutlich stärker werden, kommen Sie bitte früher in die Praxis.

Dear Mr. Vogel,
Thank you for your email and for describing your symptoms.
Mild to moderate pain after a root canal treatment is normal for the first few days. The bone at the root tip was inflamed (apical periodontitis). The treatment irritates the tissue, which is why you feel pressure when chewing.
Please take the painkiller as we discussed in the practice. If the painkiller helps and you have no fever and no marked swelling, this is usually not dangerous.
The risk of complications is higher if:
- the pain is very severe and does not improve,
- you develop a fever,
- the cheek becomes noticeably swollen,
- you can hardly open your mouth.
In these cases, please come to the practice immediately without an appointment or call the dental emergency service.
Please start the antibiotic <strong>now</strong>, as stated on the prescription: three times daily for 5 days after meals. Apply a cold pack lightly to the cheek from the outside (not directly on the skin) and continue brushing your teeth as usual. You do not need to rinse the gum additionally; normal oral hygiene is sufficient.
The drainage will remain in the tooth until the next appointment. At the follow-up next week I will take a new X-ray and assess the situation. If the pain stays the same or gradually decreases, the appointment next week is sufficient. If the pain becomes noticeably worse today or tomorrow, please come to the practice earlier.
