Sie erklärt, dass gute Mundhygiene wichtig ist, damit die Parodontitis nicht wiederkommt und das Zahnfleisch stabil bleibt.
Dr. Weber is a dentist working in a practice in Munich. Today Mr. Yilmaz comes in for a check-up. In his medical history he reports that his gums sometimes bleed.

Dr. Weber performs an examination. She measures the probing depth and finds: some teeth show signs of periodontitis. The gums are reddened and there are deep periodontal pockets.

She explains: “A routine professional tooth cleaning is prophylaxis. We remove light tartar and plaque, polish the teeth and give instructions for oral hygiene. This helps to keep the teeth healthy.”

Then she says: “But you need more. We will do scaling and root planing. That means we remove hardened tartar and deposits below the gum line. We use hand instruments and an ultrasonic scaler. We smooth the roots so that the bacteria are eliminated.”

She shows Mr. Yilmaz the correct care for home: he should brush his teeth twice a day and use interdental brushes in the evening. For tight spaces she recommends floss. She explains that good oral hygiene is important to prevent periodontitis from returning and to keep the gums stable.

Tomorrow Mr. König will come in for a check-up. Three months ago he had a periodontitis treatment with scaling and root planing.

Today I performed a professional dental cleaning for him. During the appointment I:

  • removed tartar and performed supragingival cleaning (above the gum line),
  • used the ultrasonic device and hand instruments,
  • measured pocket depths — the values have improved, but two teeth still bled on probing,
  • and finished with polishing and a fluoride treatment.

Mr. König still doesn't clearly understand the difference between a routine prophylaxis / dental cleaning and the earlier scaling and root planing. He keeps asking, “Why was the other treatment so long and uncomfortable?”

Could you please explain in simple terms to him tomorrow,

  • what the difference is between today’s dental cleaning and treatment in the deep gum pockets,
  • why subgingival cleaning (under the gum) is important,
  • and how he should adapt his oral hygiene individually (for example, interdental brushes, correct brushing technique)?

That will help him better understand the periodontal follow-up care.

Ich werde ihm morgen in einfachen Worten erklären, dass die heutige professionelle Zahnreinigung vor allem eine Reinigung über dem Zahnfleischrand ist, mit Zahnsteinentfernung, Politur und Fluoridierung. Ich sage ihm, dass diese Prophylaxe eher kurz und angenehm ist.

Dann erkläre ich, dass Scaling und Root Planing eine tiefere Behandlung ist. Dabei reinigen wir auch unter dem Zahnfleisch, in den Zahnfleischtaschen. Diese subgingivale Reinigung ist wichtig, weil dort die Bakterien für die Parodontitis sitzen. Deshalb war diese Therapie länger und manchmal unangenehm.

Für die Mundhygiene werde ich ihm eine individuell angepasste Anleitung geben. I will explain to him tomorrow in simple terms that today’s professional dental cleaning is mainly a cleaning above the gum line, including tartar removal, polishing and a fluoride treatment. I will tell him that this prophylaxis is relatively short and usually comfortable.

Then I will explain that scaling and root planing is a deeper treatment. In that procedure we also clean under the gum, in the gum pockets. This subgingival cleaning is important because the bacteria that cause periodontitis live there. That is why the earlier therapy took longer and can be uncomfortable.

For oral hygiene I will give him individualized advice. I will recommend a soft toothbrush, interdental brushes for the spaces between the teeth, and a good brushing technique twice a day. I will also explain that regular check-ups and professional dental cleanings are important to keep periodontitis stable.
