Sehr geehrte Frau Dr. Kovács,

vielen Dank für Ihre Anmeldung im nationalen Register für Zahnärzte der Landeszahnärztekammer Berlin.

Wir haben Ihre Unterlagen geprüft. Für das Zulassungsverfahren und die Anerkennung Ihrer ausländischen Abschlüsse fehlen noch einige Dokumente:

• beglaubigte Kopie Ihrer Ausbildungsnachweise (Diplom, Zeugnisse)
• aktuelles Führungszeugnis
• Gesundheitsbescheinigung (ärztliches Attest, nicht älter als 3 Monate)
• gut lesbare Passkopie
• Ihre deutsche Steueridentifikationsnummer
• Nachweis über Ihre Sozialversicherung in Deutschland

Bitte reichen Sie diese Unterlagen bis zum 30.06. per Post oder als PDF per E-Mail ein. Erst nach vollständigem Eingang können wir Ihnen die Berufserlaubnis bzw. die Approbation als Zahnarzt/Zahnärztin bescheinigen.

Außerdem senden wir Ihnen im Anhang einen Entwurf Ihres Arbeitsvertrags mit der Zahnklinik „Smile Berlin“. Der Vertrag ist zunächst für ein Jahr befristet. Die Probezeit beträgt 6 Monate. Die Kündigungsfrist in dieser Zeit liegt bei 2 Wochen.

Bitte lesen Sie den Vertrag sorgfältig. Wenn Sie Fragen zur Vergütung oder zu anderen Punkten haben, können Sie uns gerne schreiben. Wenn alles für Sie in Ordnung ist, unterzeichnen Sie bitte den Vertrag und schicken Sie ihn uns zurück.

Subject: Your registration in the dentists' register – missing documents

Dear Dr. Kovács,

thank you for your registration in the national register for dentists of the State Dental Association Berlin.

We have reviewed your documents. For the licensing procedure and the recognition of your foreign qualifications, the following documents are still missing:

• certified copy of your training certificates (diploma, transcripts)
• current certificate of conduct (police clearance)
• health certificate (medical certificate, not older than 3 months)
• clear copy of your passport
• your German tax identification number
• proof of your social insurance in Germany

Please submit these documents by 30/06 by post or as a PDF by email. Only once we have received all documents can we confirm your professional permit or your license to practise as a dentist.

In addition, we are sending you a draft of your employment contract with the dental clinic “Smile Berlin” as an attachment. The contract is initially fixed-term for one year. The probationary period is 6 months. The notice period during this time is 2 weeks.

Please read the contract carefully. If you have any questions about the remuneration or other points, please write to us. If everything is in order, please sign the contract and return it to us.
