First, the surgeon gave her assessment of the patient’s condition and clarified the risks. Then the nurse proposed a solution to improve coordination of care at home. The pharmacist explained the treatment and established the priorities for the medications. At the end, the head of department assigned the tasks and the whole team agreed on the follow-up plan in the medical record.
