  • noter clairement les entrées/sorties (I/O) sur la feuille (tout ce qu’elle boit et les pertes mesurées).

Bonjour Madame Dubois,

Pendant le repas de midi, Mme Bernard a bu environ 400 ml de liquides à texture modifiée (eau gélifiée et jus épaissi). Elle a mangé la moitié de sa purée, mais a refusé le dessert. Elle toussait un peu au début, puis la déglutition était meilleure avec la paille adaptée.

Sur la feuille d’entrées/sorties, j’ai noté tous les apports. Pour l’instant, il n’y a pas de pertes mesurées supplémentaires. Je trouve sa bouche un peu sèche, donc je vais lui proposer de petites quantités à boire plus souvent et continuer à surveiller son état d’hydratation.
