Note de service - Évaluation de la douleur et transmissions

Note de service (service de médecine) : À partir de lundi, toute douleur doit être évaluée et notée dans le dossier. Utilisez l’ de la douleur (NRS) de 0 à 10 et signalez aussi les (grimace, protection d’une zone, rythme respiratoire). Pour les douleurs chroniques, précisez l’impact sur le et sur les quotidiennes.

Pour les transmissions, appliquez la méthode : situation, contexte, évaluation, recommandation. Si vous signalez un changement clinique, joignez la de la douleur et, si nécessaire, les . Objectif : une prise en charge centrée sur la , en veillant au confort du patient.
Memo (internal medicine ward): Starting Monday, all pain must be assessed and recorded in the chart. Use the numeric pain rating scale (NRS) from 0 to 10 and also report nonverbal indicators (grimacing, guarding an area, respiratory rate). For chronic pain, specify the impact on sleep and on daily activities.

For handovers, apply the SBARR method: situation, background, assessment, recommendation. If you report a clinical change, attach the WHO pain classification and, if necessary, Gordon’s functional health patterns. Goal: care centered on positive health, while ensuring the patient’s comfort.

I’m leaving you an update before handover for Mr Benali (room 312). Since 2 p.m., he has been complaining of abdominal pain. At 4 p.m., NRS 7/10, tense face and he often grimaces. He says it hurts mainly when he moves. Paracetamol given at 4:10 p.m., little effect at 5 p.m. (NRS 6). BP 128/78, HR 96, T° 37.4, respiratory rate 22/min. No nausea. Can you reassess and report any change to the on-call doctor?

SBARR :
- Situation : douleur abdominale persistante.
- Contexte : douleur surtout à la mobilisation, paracétamol à 16h10 avec effet limité.
- Évaluation : NRS 6/10 au repos, 7/10 à la mobilisation. Il pose la main sur le ventre, fait de courtes grimaces et respire plus vite (RR 22-24/min). TA 130/80, FC 98, T° 37,5. Pas de nausées ni de vomissements.
- Recommandation : prévenir le médecin de garde pour avis sur l'antalgie et envisager un autre traitement si nécessaire. Je poursuis la surveillance et note tout changement dans le dossier.

Je te tiens informée en cas d'évolution.
