(Hello Mr. Martin, I’m doing a quick assessment: since this morning, have you been in a rather stable condition, or do you feel a flare-up?)

Tu me dis après ton passage ? Merci.

Hi, it’s Clara (RN). Can you come and see Mr. Martin in room 12? He has COPD and heart failure. Since this morning, he’s not as well: more short of breath and he’s coughing more. His oxygen saturation has dropped to 90% on room air. He also says his ankles are swollen.

The doctor wants us to monitor symptoms and note whether it’s an exacerbation or a decompensation. Can you let me know after you’ve seen him? Thanks.

Useful phrases:

(I’m going to room 12 right away and I’ll keep you updated.)

(I’m going to monitor the oxygen saturation, breathing, and ankle swelling.)

(Has he had oxygen therapy or a change in treatment this morning?)

Hi Clara, I’m going to see Mr. Martin now. I’ll check the oxygen saturation, respiratory rate, cough, and examine his ankles for swelling. I’ll also ask him since when he has been more short of breath and whether he has any pain. If the saturation stays at 90% or if his condition worsens, I’ll inform the doctor. After the assessment, I’ll tell you whether it looks more like a COPD exacerbation or cardiac decompensation, and I’ll document everything in the chart.
