Please record the amounts consumed on the “Intake and output” sheet and complete the fluid balance at the end of the shift. Monitor for signs of dehydration: dry mouth, fatigue, dark urine. If she drinks little despite assistance with drinking, inform the nurse in charge.

Here is a quick update on Mr. Delmas (214). At breakfast, he coughed while drinking water: suspected dysphagia. We switched to a modified texture (purée) and limited thin liquids: thickened drink, small sips, seated posture with the head slightly elevated.
Could you, this afternoon, record the amounts taken in and complete the fluid balance chart (input/output)? Also monitor signs of dehydration (dry mouth, dark urine). Thank you.
