Note de transmission - chambre 12

Note de transmission (service de gériatrie)

Handover note (geriatrics unit)

This morning, I monitored Mrs. L. She is less alert than usual and responds slowly. Her skin is paler and she has chills. I measured the temperature and pulse: the temperature is high and the pulse is fast.

She is breathing faster and complains of fatigue. I noted my observations in the chart. Please check the blood pressure and oxygen saturation, and report any abnormality to the nurse in charge.

(What signs show that Mrs. L.'s condition has changed and what should the person reading this note do?)

Hello, it’s Sophie (lead RN). Can you give me a quick update on Mr. Durand, room 12?

  • Vitals at 2 p.m.: temperature, pulse, blood pressure, oxygen saturation, respiration.
  • His condition: is he alert? Is he responding well?
  • Did you note anything unusual: skin color, cough/sputum, amount of urine?

Thanks, I have to write the written report for the doctor.
