Note interne du cadre de santé : Depuis quelques jours, nous avons eu deux situations de aux urgences. Chez certains patients, des et des peuvent provoquer de la peur et de l’agitation. Si vous observez une , une anxiété sévère ou un discours incohérent, notez l’heure et prévenez l’infirmier référent.
Pendant l’accueil, restez calme, parlez lentement et gardez une distance de sécurité. Ne validez pas les idées du patient, mais reconnaissez son stress. Cherchez des : menaces, gestes brusques, refus total de soins. En cas de risque, la situation avec le médecin et le patient vers l’équipe psychiatrique. Pour un patient avec (par exemple le syndrome de Rett), expliquez avec des phrases courtes et utilisez des repères simples.Internal memo from the health unit manager: Over the past few days, we have had two situations of acute psychosis in the emergency department. In some patients, hallucinations and delusions can cause fear and agitation. If you observe disorientation, severe anxiety, or incoherent speech, note the time and inform the responsible nurse.
During admission, stay calm, speak slowly, and keep a safe distance. Do not validate the patient’s ideas, but acknowledge their stress. Look for crisis warning signs: threats, sudden gestures, total refusal of care. If there is a risk, assess the situation with the doctor and refer the patient to the psychiatric team. For a patient with an intellectual disability (for example Rett syndrome), explain with short sentences and use simple cues.

Je te fais un point rapide sur M. Lemaire (ch. 12). Depuis 5 h, il est très désorienté, dit entendre des voix (hallucinations) et affirme que « le voisin l’espionne » (propos délirants). Il refuse le petit-déjeuner et tourne dans le couloir. Pour l’instant il n’a pas frappé, mais il est de plus en plus tendu.
J’ai essayé de le calmer en parlant doucement et je l’ai installé près du poste. Peux-tu passer dès ton arrivée et me dire si on évalue le risque avec le médecin de garde / psy, ou si on l’oriente vers une équipe spécialisée ?

Hello,
I’m giving you a quick update on Mr. Lemaire (rm. 12). Since 5 a.m., he has been very disoriented, says he hears voices (hallucinations) and claims that “the neighbor is spying on him” (delusional statements). He is refusing breakfast and pacing in the hallway. So far he hasn’t hit anyone, but he is getting increasingly tense.
I tried to calm him down by speaking softly and I seated him near the nurses’ station. Can you come by as soon as you arrive and tell me whether we should assess the risk with the on-call doctor / psychiatrist, or whether we should refer him to a specialized team?

Peux-tu me dire s’il a déjà eu ce type d’épisode (psychose aiguë / épisode psychotique) pendant l’hospitalisation et s’il a pris son traitement cette nuit ? Est-ce qu’il a menacé quelqu’un ou tenté de sortir du service ?
Je préviens le médecin de garde à 7 h 30 pour qu’on évalue le risque et décide s’il faut appeler la psy ou une équipe spécialisée. En attendant, on le garde près du poste et on surveille étroitement.

Can you tell me whether he has already had this type of episode (acute psychosis / psychotic episode) during this hospitalization and whether he took his medication tonight? Has he threatened anyone or tried to leave the unit?
I will notify the on-call doctor at 7:30 a.m. so that we can assess the risk and decide whether we need to call the psychiatrist or a specialized team. In the meantime, we’ll keep him near the nurses’ station and monitor him closely.
