Intranetbericht: nieuw format voor pijnbeoordeling en overdracht

Vanaf volgende week gebruikt afdeling Chirurgie één vast format voor . Noteer bij elke ronde de pijnscore op de , de lokalisatie en de duur, en beschrijf ook zoals gezichtsuitdrukking en lichaamstaal. Als de patiënt niet goed kan praten, kies je een passend en leg je kort uit waarom.

In de overdracht gebruiken we voortaan : situatie, achtergrond, beoordeling, aanbeveling en reactie. Bij de beoordeling koppel je de pijn aan de en aan functionele beperkingen (bijvoorbeeld slapen of mobiliteit), volgens Gordon’s functionele patronen. In het zorgplan vermeld je ook één punt uit , zoals wat de patiënt nog wél kan.
Starting next week, the Surgery department will use one fixed format for pain assessment. At each round, record the pain score on the NRS, the location and the duration, and also describe non-verbal signs such as facial expression and body language. If the patient cannot speak well, choose an appropriate pain measurement instrument and briefly explain why.

In the handover we will from now on use SBARR: situation, background, assessment, recommendation and response. In the assessment you link the pain to the WHO classification and to functional limitations (for example sleeping or mobility), according to Gordon’s functional patterns. In the care plan you also mention one point from Positive Health, for example what the patient can still do.

Hoi Sanne,

Ik heb net bij meneer De Vries (kamer 14) de pijn beoordeeld. NRS in rust 3/10, bij draaien in bed 7/10. De pijn zit vooral rechts onder rond de wond en is sinds vanochtend erger, vooral bij bewegen. Ik zag non-verbale tekenen: hij trok zijn gezicht samen, hield zijn buik vast en ademde sneller.

SBARR:
- Situatie: toename van pijn bij mobiliseren.
- Background: postoperatief, pijnmedicatie volgens schema.
- Assessment: NRS 7 bij bewegen; hierdoor lukt wassen en uit bed komen moeilijker (functiebeperking/ADL).
- Recommendation: graag evaluatie van pijnstilling (mogelijk extra medicatie of andere maatregel) en advies voor veilig mobiliseren. Ik blijf NRS en effect van medicatie monitoren en noteer dit in het dossier.

Hi Sanne,

I have just assessed Mr De Vries (room 14)’s pain. NRS at rest 3/10, when turning in bed 7/10. The pain is mainly on the lower right around the wound and has been worse since this morning, especially with movement. I observed non-verbal signs: he grimaced, held his abdomen, and breathed faster.

SBARR:
- Situation: increase in pain when mobilising.
- Background: postoperative, pain medication as scheduled.
- Assessment: NRS 7 with movement; because of this, washing and getting out of bed is more difficult (functional limitation/ADL).
- Recommendation: please evaluate pain relief (possibly additional medication or another measure) and advice for safe mobilisation. I will continue to monitor the NRS and the effect of the medication and record this in the chart.
