Op de gesloten afdeling van een psychiatrisch ziekenhuis begint de avonddienst. Verpleegkundige Sara leest in het elektronisch dossier dat meneer Van Dijk heeft en de laatste weken vaker stemmen hoort. In het staat dat zijn wisselt: soms kan hij goed vertellen wat er gebeurt, maar soms denkt hij dat de tv speciaal voor hem geheime berichten uitzendt. Sara let extra op , zoals plotselinge boosheid, of weigeren van medicatie.
Tijdens de ronde ziet Sara dat meneer Van Dijk onrustig door de gang loopt. Hij zegt dat de camera’s hem volgen en dat iemand hem wil vergiftigen. Sara blijft rustig en spreekt in korte, duidelijke zinnen. Ze gaat niet in discussie over zijn , maar benoemt wat zij ziet: dat hij bang is en veel spanning voelt. Ze controleert of hij veilig is, biedt een rustige kamer aan en bespreekt het dat in zijn staat. Daarna belt ze met de arts om de situatie te bespreken en om te beoordelen of extra of aanpassing van de nodig is.On the locked ward of a psychiatric hospital, the evening shift begins. Nurse Sara reads in the electronic record that Mr. Van Dijk has schizophrenia and has been hearing voices more often in recent weeks. The observation report notes that his sense of reality fluctuates: sometimes he can clearly describe what is happening, but other times he believes the TV is broadcasting secret messages specifically for him. Sara pays extra attention to crisis signs, such as sudden anger, self-harm, or refusal of medication.
During her round, Sara sees Mr. Van Dijk pacing restlessly down the corridor. He says the cameras are following him and that someone wants to poison him. Sara remains calm and speaks in short, clear sentences. She does not argue about his delusions, but states what she observes: that he is frightened and very tense. She checks whether he is safe, offers a quiet room, and goes over the safety plan that is part of his treatment plan. Afterwards she calls the physician to discuss the situation and to assess whether additional interventions or an adjustment of his medication are necessary.
