Tijdens de nachtdienst op een somatische afdeling vraagt mevrouw De Boer om hulp bij het toilet. Ze is net geopereerd aan haar heup en mag nog niet zelf uit bed komen. U helpt haar met uitkleden en legt rustig uit dat u een gebruikt. U controleert of de rem van het bed vaststaat en haar voorzichtig bij het draaien. Daarna laat u haar in rust en . U blijft in de buurt, maar geeft haar zoveel mogelijk privacy.

Na afloop helpt u haar schoonmaken en legt u een schoon aan. U de urine en de ontlasting. De urine heeft een lichtgele kleur en een normale . De ontlasting is zacht, maar geen . U noteert in het elektronisch dossier: kleur, , hoeveelheid en of er bijzondere geur is. Ook schrijft u dat mevrouw lichte heeft gemeld. Daarna u de bedpan, gooit de inhoud op de juiste manier weg en meldt kort uw observaties aan de dienstdoende arts-assistent tijdens de overdracht.
During the night shift on a somatic ward, Mrs De Boer asks for help with toileting. She has just had hip surgery and is not yet allowed to get out of bed by herself. You help her undress and calmly explain that you will use a bedpan. You check that the bed brake is engaged and gently support her while turning. Then you let her urinate and have a bowel movement in peace. You stay nearby but give her as much privacy as possible.

Afterwards you help her clean up and put on a clean incontinence pad. You observe the urine and the stool. The urine is light yellow and present in a normal amount. The stool is soft but not diarrhoea. You record in the electronic file: colour, consistency, amount and whether there is any unusual odour. You also note that Mrs De Boer reported mild gastrointestinal complaints. Then you disinfect the bedpan, dispose of the contents correctly and briefly report your observations to the on-duty resident during handover.

Hierbij mijn rapportage over meneer Van Dijk van vannacht.

Rond 02.00 uur heb ik hem geholpen met de urinaal. Hij had normale hoeveelheid urine, lichtgeel van kleur, niet erg donker. Er was geen pijn en geen bijzondere geur.

Tegen 05.30 uur heb ik hem op de postoel gezet. Hij had een kleine hoeveelheid ontlasting, bruine kleur en normale consistentie, geen diarree en geen bloed.

Na de stoelgang heb ik zijn schaamstreek gewassen, goed afgedroogd en nieuw incontinentiemateriaal omgedaan. De huid zag er niet rood of geïrriteerd uit.

Here is my report on Mr. Van Dijk from last night.

At around 02:00 I helped him with the urinal. He produced a normal amount of urine, light yellow in color, not dark. He did not report any pain and there was no unusual odor.

At about 05:30 I placed him on the commode. He had a small bowel movement, brown in color and of normal consistency; no diarrhea and no blood.

Afterwards I washed his genital area, thoroughly dried it and put on fresh incontinence material. The skin did not look red or irritated.
