Intranetbericht: nieuwe werkwijze toiletassistentie
Vanaf volgende week gebruiken we op afdeling 3 één vaste werkwijze bij toiletassistentie. Gebruik een of als de patiënt niet veilig naar het toilet kan. Werk hygiënisch: aan, huid wassen en goed drogen, en daarna een schone of een schoon klaarzetten.
Rapporteer na elk zorgmoment kort in het dossier: de kleur en hoeveelheid van de en hoe de eruitzag. Meld dit direct bij donkergele urine, bij of bij . Zo kunnen we snel overleggen met de arts.Starting next week, we will use one fixed procedure for toileting assistance on ward 3. Use a urinal or bedpan if the patient cannot safely go to the toilet. Work hygienically: put on gloves, wash the skin and dry it well, and then prepare a clean diaper or a clean collection container.
After each care moment, briefly report in the file: the color and amount of urine and what the stool looked like. Report this immediately in case of dark yellow urine, diarrhea, or constipation. This way we can quickly consult with the doctor.

Sanne (verpleegkundige): Show Ik heb meneer De Vries net verschoond: een schone luier aangetrokken, zijn huid gewassen en droog gedept met steriele handschoentjes, en daarna voorzichtig gepoetst omdat de huid wat rood was.

Korte overdracht over mevrouw De Vries (kamer 12). Vanmiddag had ze meerdere keren aandrang, maar ze haalde het toilet niet altijd. We hebben haar 2x met het urinaal geholpen en 1x met de bedpan. De urine was donkergeel, hoeveelheid ongeveer 200-250 ml per keer. Ze had ook ontlasting: papperig, niet veel. De huid rond het zitvlak is wat rood; na afloop gewassen en goed gedroogd.

Brief handover about Mrs. De Vries (room 12). This afternoon she had the urge several times, but she didn't always make it to the toilet. We helped her 2x with the urinal and 1x with the bedpan. The urine was dark yellow, amount about 200-250 ml each time. She also had stool: mushy, not much. The skin around the buttocks is a bit red; washed afterward and dried well.

Dank je voor je duidelijke overdracht. Ik ben morgenochtend vroeg aanwezig en plan vaste momenten om mevrouw De Vries te helpen met plassen. Als ze het toilet niet haalt, gebruik ik eerst het urinaal en anders de bedpan. Ik let extra op de kleur en hoeveelheid van de urine en op de consistentie van de ontlasting en noteer dit in het dossier.
Voor de rode huid was ik na elke verschoning, droog ik goed en breng ik een beschermende crème aan volgens protocol.

If she doesn’t make it to the toilet, I will use the urinal first and otherwise the bedpan. For the red skin, I will wash after every change, dry thoroughly, and apply a protective cream according to protocol.
