Op het intranet van Thuiszorg Noord staat een bericht over mevrouw De Vries. Zij krijgt hulp van haar dochter, die is. De wijkverpleegkundige merkt dat de groter wordt. Daarom komt er één voor een wandeling en een praatmoment, één keer per week. De familie wil graag weten wie welke taken uitvoert en wanneer.
Teamleden moeten vandaag de invullen in het dossier. Maak ook korte na elk bezoek en geef belangrijke informatie door aan de mantelzorger, maar bewaar de . Hierbij de aanvulling voor mevrouw De Vries.
- Vrijwilliger: komt dinsdagmiddag één uur voor gezelschap. Activiteiten: samen koffie drinken, krant lezen en een korte wandeling als mevrouw zich goed voelt. De vrijwilliger doet geen ADL-zorg en geeft geen medicatie.
- Toestemming: in het dossier staat een toestemmingsverklaring voor overleg met de dochter, ondertekend vorige maand. Voor de vrijwilliger is er nog geen specifieke toestemming; daarom deel ik alleen praktische afspraken en geen medische gegevens.
- Observaties: mevrouw is ’s ochtends soms wat verward maar overdag rustig en coöperatief. Ze eet iets minder dan normaal en klaagde twee keer over knie‑pijn na het lopen. Ik heb aantekeningen gemaakt in het verslag.
In de zorgplanning heb ik toegevoegd dat we na elk bezoek kort evalueren met de mantelzorger en dat we duidelijke grenzen aangeven richting de vrijwilliger (alleen gezelschap, geen zorgverlening).
Vraag: wil je dat ik de takenlijst voor de vrijwilliger ook apart in het dossier zet, of is een e-mail aan jou genoeg?

Hi Sanne,
Here is the addition for Mrs De Vries.
- Volunteer: comes Tuesday afternoon for one hour for companionship. Activities: having coffee together, reading the newspaper and a short walk if Mrs feels well. The volunteer does not provide ADL care and does not administer medication.
- Consent: the file contains a consent form for consultation with the daughter, signed last month. For the volunteer there is not yet specific consent; therefore I only share practical arrangements and no medical data.
- Observations: in the mornings Mrs is sometimes a bit confused, but during the day she is calm and cooperative. She eats a little less than normal and complained twice about knee pain after walking. I made notes in the report.
In the care plan I added that after each visit we briefly evaluate with the informal caregiver and that we indicate clear boundaries towards the volunteer (companionship only, no caregiving).
Question: would you like me to also put the task list for the volunteer separately in the file, or is an email to you enough?
