Op onze afdeling controleren we elke dag de van cliënten die veel in bed of in een stoel liggen. Let extra op : hielen, stuit en ellebogen. Kijk of er is en of de plek warm aanvoelt. Vraag ook of de plek doet. Bij een beginnende blijft de roodheid vaak zichtbaar als je er kort op drukt.

Is de huid kapot of zie je een met korst of ? Maak de plek dan schoon volgens het en houd de huid droog. Breng zo nodig een nieuw aan. Meld altijd in het dossier: plaats, kleur, grootte en of de huid ontstoken of lijkt. Noteer ook het , bijvoorbeeld bij weinig bewegen, ondergewicht of een vochtige huid.
On our ward we check the skin of clients who spend a lot of time in bed or in a chair every day. Pay extra attention to pressure points: heels, tailbone and elbows. Look for redness and whether the area feels warm. Also ask whether the area is painful. With an early pressure sore the redness often remains visible when you press it briefly.

Is the skin broken or do you see a wound with a crust or discharge? Clean the area according to the care plan and keep the skin dry. Apply a fresh dressing if needed. Always record in the file: location, colour, size and whether the skin appears inflamed or infected. Also note risk factors, for example reduced mobility, underweight or moist skin.
