(Mr De Vries, is it okay if we briefly have a conversation about your wishes for the time ahead?)
Meneer De Vries (patiënt): Show Ja… ik wil vooral rust.
(Yes… above all I want peace and quiet. I want palliative care and preferably no resuscitation anymore, so a do-not-resuscitate order, please.)
(Thank you for telling me. I understand and I respect your wishes. Do you also have an advance directive that we can put in your file?)
Meneer De Vries (patiënt): Show Ja, die ligt thuis.
(Yes, it’s at home. My daughter can bring it. And I would like you to take my faith into account when saying goodbye.)
Verpleegkundige Sara: Show Dat is goed.

Groet,
Samira
Hi Inge,
Yes, I can join the conversation at 15:30 in room 12.
I will check the file beforehand to see whether there is an advance directive or a do-not-resuscitate order. If that is not clear, I will briefly consult with the doctor before the conversation.
During the conversation, I mainly want to listen and show understanding. I will ask what Mr. De Vries himself wants and what the daughter finds important when saying goodbye. I will also kindly ask about the religious ritual, so that we can take it into account.
Kind regards,
Samira
