Ik ben fysiotherapeut op afdeling 3B. Ik wil graag met jou de revalidatie van meneer Jansen (kamer 312) bespreken. Meneer had drie weken geleden een beroerte (CVA). Hij heeft nu een lichte parese in de rechterarm en -been en soms een klein spraakprobleem.
Voor de volgende evaluatie van het revalidatiedoel heb ik de volgende vragen:
- Hoe gaat het nu met zijn mobiliteit in bed (bedmobiliteit)? Kan hij zich al zelf een beetje omrollen en zitten op de bedrand?
- Welke hulpmiddelen gebruiken jullie nu bij de transfers (bijvoorbeeld bij opstaan uit bed en naar de rolstoel)?
- Heb je valpreventie-problemen gezien? Is hij al eens bijna gevallen?
Ik wil dit gebruiken voor de nieuwe zorgplanning en het zorgleefplan. Kun je mij vóór donderdag een korte terugkoppeling mailen voor de verpleegkundige overdracht? Dan kunnen we samen kijken hoe we hem verder kunnen ondersteunen en herstellen kunnen verbeteren.

I am a physiotherapist on ward 3B. I would like to discuss the rehabilitation of Mr. Jansen (room 312) with you. Mr. Jansen had a stroke (CVA) three weeks ago. He now has a mild paresis in the right arm and leg and sometimes a slight speech problem.
For the next evaluation of the rehabilitation goal I have the following questions:
- How is his mobility in bed (bed mobility) now? Can he already turn over a little by himself and sit on the edge of the bed?
- Which aids are you currently using for the transfers (for example when getting up from bed and to the wheelchair)?
- Have you observed any fall risk issues? Has he nearly fallen at any time?
I want to use this for the new care plan and the individual care plan. Could you email me brief feedback before Thursday for the nursing handover? Then we can discuss how best to support him and improve his recovery.

Bedankt voor uw e-mail over de revalidatie van meneer Jansen.
Zijn bedmobiliteit gaat langzaam beter. Hij kan zich met hulp een beetje omrollen naar links en rechts. Met de bedrand omhoog kan hij kort zelfstandig zitten op de bedrand, maar hij wordt snel moe.
Bij de transfers gebruiken wij nu een rollator en een hoge toiletverhoger. Bij opstaan uit bed ondersteunen wij hem met twee personen. Wij letten op een veilige tiltechniek en we geven duidelijke instructies. Naar de rolstoel gaat hij nog niet alleen, altijd met begeleiding.
Tot nu toe is hij niet gevallen. Eén keer is hij bijna gevallen bij het lopen naar het toilet. Daarom zetten wij nu de rolstoel dichter bij het bed en herinneren wij hem om op de bel te drukken als hij wil opstaan. Valpreventie blijft belangrijk.
Mijn advies is om verder te oefenen met veilig opstaan en zitten, steeds met dezelfde hulpmiddelen. Misschien kunnen we samen met u een vast transferschema maken.

Dear Ms. De Vries,
Thank you for your email about Mr. Jansen's rehabilitation.
His bed mobility is slowly improving. With assistance he can turn slightly to the left and right. With the bedrails lowered he can sit on the edge of the bed for a short time independently, but he tires quickly.
For transfers we are currently using a walker and a raised toilet seat. When getting up from bed we assist him with two staff members. We ensure safe handling techniques and give clear instructions. He does not go to the wheelchair on his own yet; he always needs supervision.
So far he has not fallen. He nearly fell once while walking to the toilet. Because of that, we now place the wheelchair closer to the bed and remind him to press the call bell when he wants to get up. Fall prevention remains important.
My advice is to continue practising safe rising and sitting, always using the same aids. Perhaps we can draw up a fixed transfer schedule together with you.
