It is 10:30 PM on the internal medicine ward. The evening team is handing over to the night shift. The nurse on duty, Eva, reads Mr. Van Dijk's report.

The report says: “Patient alert but tired. Condition stable. Vital signs good. Blood pressure 130/80, heart rate 78, breathing unlaboured. Temperature 37.4°C. No new complaints reported. Medication given as scheduled. No side effects observed.”

Eva says to her colleague on the night shift: “Could you check his blood pressure again at the start of your shift? It was a bit higher yesterday. Also keep an eye out for any new complaints, such as chest pain or shortness of breath. If the blood pressure rises quickly or the patient suddenly becomes dizzy, call the doctor immediately.”

The colleague nods and writes in her own note: “Extra blood pressure check at the start of the shift. Inform the doctor immediately if the condition changes.”
