Intranetbericht: extra monitoring bij chronische ziekten

Op de afdeling letten we deze week extra op cliënten met een , zoals , of . Let op veranderingen in symptomen: meer kortademigheid, dikke enkels of een snelle gewichtstoename. Dat kan passen bij linker- of rechterzijdige decompensatie. Noteer alles in het behandelplan en overleg bij twijfel met de huisarts.

Als de cliënt stabiel is, gaan de ademhalingsoefeningen door. Bij een of veel kan het nodig zijn om medicatie aan te passen. Schat de zorgbehoefte opnieuw in en maak een monitoringafspraak voor morgen.
On the ward, this week we are paying extra attention to clients with a chronic illness, such as diabetes, COPD or heart failure. Watch for changes in symptoms: more shortness of breath, swollen ankles or rapid weight gain. That can be consistent with left- or right-sided decompensation. Record everything in the treatment plan and, if in doubt, consult the family doctor.

If the client is stable, the breathing exercises will continue. In the event of an exacerbation or a lot of fluid retention, it may be necessary to adjust medication. Reassess the care needs and schedule a monitoring appointment for tomorrow.

I am with Mr De Vries (room 12). He has COPD and heart failure. Since last night he has been more short of breath. His ankles are more swollen (possible fluid retention). He is now sitting upright in bed and says he gets tired more quickly. Temperature is 37.4 °C. Saturation is 91% with 2 L oxygen. He is using his inhalation medication, but it helps little.

Could you think along with me? Is this an exacerbation or is his condition changing? What do we agree on: extra monitoring, adjust medication via the doctor, or refer to the GP?

Dank je voor de update. Ik denk dat dit kan passen bij een verergering van zijn hartfalen of een COPD-exacerbatie, omdat hij meer kortademig is en vochtretentie heeft.

Ik kom nu langs voor extra monitoring: ademhaling, saturatie, pols en bloeddruk. Ik help hem rechtop zitten en doe korte ademhalingsoefeningen. Daarna bel ik de dienstdoende arts om het behandelplan te bespreken en te overleggen of we medicatie moeten aanpassen (bijvoorbeeld diureticum of extra inhalatie). Als hij achteruitgaat, verwijzen we direct.

Thanks for the update. I think this could fit with a worsening of his heart failure or a COPD exacerbation, because he is more short of breath and has fluid retention.

I’ll come by now for extra monitoring: breathing, saturation, pulse, and blood pressure. I’ll help him sit upright and do short breathing exercises. After that I’ll call the on-call doctor to discuss the treatment plan and to consult whether we need to adjust medication (for example a diuretic or extra inhalation). If he deteriorates, we will refer immediately.
