(Observation on the ward)
Sara is verpleegkundige in een ziekenhuis in Den Haag. Ze werkt op een interne afdeling. Aan het begin van de dienst leest ze eerst de dossiers van de patiënten. Daarna gaat ze langs de kamers om de patiënten te observeren.
Bij meneer De Vries meet Sara de bloeddruk, de lichaamstemperatuur, de hartslag en de ademhaling. De hartslag is wat snel, maar regelmatig. De ademhaling is rustig. De saturatie is goed. Meneer De Vries is alert en praat rustig met Sara.
Daarna gaat Sara naar mevrouw Janssen. Mevrouw kijkt suf en reageert langzaam. Haar huid voelt koud aan en haar gezicht is erg bleek. De ademhaling is onregelmatig en de ademhalingsfrequentie is hoog. Ook de pols is snel. Sara vindt dit veranderd gedrag.
Sara schrijft alle waarden in het dossier. Ze zorgvuldig de tijd, de , de , de en de . Dan belt ze meteen haar en haar observaties: mevrouw is suf, voelt koud aan en ademt onregelmatig. De supervisor komt direct naar de kamer om samen verder te kijken.Sara is a nurse in a hospital in The Hague. She works on a medical ward. At the start of her shift she first reads the patients' records. Then she goes around the rooms to observe the patients.
In Mr. De Vries's room Sara measures blood pressure, body temperature, heart rate and respiration. The heart rate is slightly fast but regular. Breathing is calm. Oxygen saturation is good. Mr. De Vries is alert and speaks calmly with Sara.
Then Sara goes to Mrs. Janssen. Mrs. Janssen looks drowsy and responds slowly. Her skin feels cold and her face is very pale. Breathing is irregular and the respiratory rate is high. The pulse is also fast. Sara considers this a change in behaviour.
Sara records all values in the record. She carefully notes the time, the blood pressure , the heart rate , the respiration and the body temperature . She then immediately calls her supervisor and reports her observations: Mrs. Janssen is drowsy, feels cold and breathes irregularly. The supervisor comes straight to the room to assess the situation together.
