Op de afdeling komt vandaag mevrouw De Vries terug na een . In de staat: links wat en soms spasticiteit. Let op bij het in bed. Bij plots duizelig worden of een scheve mond: direct de arts bellen.
Om 14.00 uur is het met fysiotherapie. Het hersteldoel deze week: veilig overzetten van bed naar rolstoel met verpleegkundige. Daarna oefenen we met een loophulpmiddel. Noteer kort hoe de mobiliteit gaat en welke ondersteuning nodig is.In the rehabilitation department, Mrs De Vries is returning today after a stroke. The handover notes: some paralysis on the left and sometimes spasticity. Be careful when turning over in bed. If she suddenly becomes dizzy or has a crooked mouth: call the doctor immediately.
At 14:00 there is the evaluation meeting with physiotherapy. The recovery goal this week: safely transfer from bed to wheelchair with one nurse. After that we practise with a walking aid. Briefly note how mobility is going and what support is needed.

Transfer van bed naar rolstoel
Verpleegkundige (Sanne): Show Goedemorgen meneer Van Dijk, we gaan nu van bed naar de rolstoel. Kunt u eerst even omrollen naar uw rechterzij?

Patiënt (Meneer Van Dijk): Show Ja, maar mijn linkerarm is slap sinds de beroerte, dat voelt als verlamming.

Verpleegkundige (Sanne): Show Ik ondersteun u bij de transfer: zet uw voeten op de grond, leun met uw neus naar uw knieën en dan staan we samen op. Ik til niet, we doen het veilig en volgens het zorgplan.

Morgen om 10:30 heb ik een kort evaluatiegesprek met mevrouw De Wit (kamer 14). Zij is na een beroerte links wat verlamd en heeft soms spasticiteit in haar arm. We oefenen nu met zich verplaatsen in de kamer en met een loophulpmiddel (wandelstok). Kun jij erbij zijn? Ik wil ook afstemmen hoe het gaat met omrollen in bed en het overzetten naar de rolstoel, zodat de overdracht duidelijk is.

Aandachtspunt 1: Bij het overzetten naar de rolstoel zet ik de rolstoel dichtbij en op de rem. Ik ondersteun haar aan de linkerkant en vraag haar rustig te gaan staan.
Aandachtspunt 2: Bij omrollen in bed werken we stap voor stap en stoppen we als de spasticiteit toeneemt. Geef duidelijke instructies tijdens de transfer.
Kun je in de overdracht noteren dat mevrouw de wandelstok alleen met begeleiding gebruikt?

Point of attention 1: When transferring to the wheelchair, I position the wheelchair close by and apply the brakes. I support her on the left side and ask her to stand up calmly.

Could you note in the handover that Ms De Wit only uses the walking stick with supervision?
