Samira is verpleegkundige op de van een ziekenhuis. Zij zorgt voor meneer Van Dijk. Hij herstelt van een lichte beroerte. Hij heeft nog in zijn rechterarm en hij loopt onzeker.

We passen het plan aan als de situatie verandert.”
Samira is a nurse on the rehabilitation ward of a hospital. She cares for Mr Van Dijk. He is recovering from a minor stroke. He still has weakness in his right arm and walks unsteadily.

Today Samira has a short evaluation meeting with the physiotherapist and with Mr Van Dijk. The physiotherapist says: “His mobility is better. He can now get out of bed by himself and stand briefly with the walker. His balance is not yet stable. We must keep practicing every day.”

Together they look at the rehabilitation goal: safely moving from bed to chair and walking short distances in the corridor. Samira says: “When turning in bed he still needs help. Also when transferring to the wheelchair I have to support him properly. He gets tired quickly, but he has little pain.”

They agree on a new transfer plan. First Mr Van Dijk sits on the edge of the bed. Then he places his feet correctly. Samira checks the brakes of the wheelchair. Then she helps him stand up slowly. Finally he turns slowly and sits down carefully. At the end of the conversation they say: “Safety is the most important. We will adjust the plan if the situation changes.”

Beste mevrouw De Vries,

Bedankt voor uw e-mail over de revalidatie van meneer Jansen.

Zijn bedmobiliteit gaat langzaam beter. Hij kan zich met hulp een beetje omrollen naar links en rechts. Met de bedrand omhoog kan hij kort zelfstandig zitten op de bedrand, maar hij wordt snel moe.

Bij de transfers gebruiken wij nu een rollator en een hoge toiletverhoger. Bij opstaan uit bed ondersteunen wij hem met twee personen. Wij letten op een veilige tiltechniek en we geven duidelijke instructies. Naar de rolstoel gaat hij nog niet alleen, altijd met begeleiding.

Tot nu toe is hij niet gevallen. Eén keer is hij bijna gevallen bij het lopen naar het toilet. Daarom zetten wij nu de rolstoel dichter bij het bed en herinneren wij hem om op de bel te drukken als hij wil opstaan. Valpreventie blijft belangrijk.

Mijn advies is om verder te oefenen met veilig opstaan en zitten, steeds met dezelfde hulpmiddelen. Misschien kunnen we samen met u een vast transferschema maken.

Dear Ms. De Vries,

Thank you for your email about Mr. Jansen's rehabilitation.

His bed mobility is slowly improving. With assistance he can turn slightly to the left and right. With the bedrails lowered he can sit on the edge of the bed for a short time independently, but he tires quickly.

For transfers we are currently using a walker and a raised toilet seat. When getting up from bed we assist him with two staff members. We ensure safe handling techniques and give clear instructions. He does not go to the wheelchair on his own yet; he always needs supervision.

So far he has not fallen. He nearly fell once while walking to the toilet. Because of that, we now place the wheelchair closer to the bed and remind him to press the call bell when he wants to get up. Fall prevention remains important.

My advice is to continue practising safe rising and sitting, always using the same aids. Perhaps we can draw up a fixed transfer schedule together with you.
