Ik heb net in het dossier gekeken. De verwijzing naar cardiologie staat als plan in het artsenverslag, maar ik zie nog geen verwijsbrief van de huisarts als bijlage. Ik bel meteen met de huisartspraktijk om te vragen of zij de verwijsbrief vandaag kunnen sturen of direct naar de poli kunnen mailen.
Wat betreft de zorg na ontslag: dhr. Van Dijk woont alleen en is bij inspanning nog snel benauwd. Mijn inschatting is dat wijkverpleging (extramurale zorg) nodig is voor medicatiecontrole en hulp bij ADL. Ik stel voor dat we een eerste huisbezoek binnen 24 uur plannen (outreach). Een tijdelijke opname in een verpleeghuis (intramurale zorg) raad ik alleen aan als zijn mobiliteit of zelfzorg niet verbetert of als er geen mantelzorg beschikbaar is.
Kun jij nagaan of de zorgverzekeraar voorwaarden heeft voor wijkverpleging (bijv. contract of eigen risico), zodat we dit met de patiënt kunnen bespreken?

Dear Marieke,
I’ve just looked in the file. The referral to cardiology is listed as a plan in the physician’s report, but I don’t yet see a referral letter from the GP as an attachment. I will call the GP practice right away to ask whether they can send the referral letter today or email it directly to the outpatient clinic.
Regarding care after discharge: Mr. Van Dijk lives alone and still becomes short of breath quickly with exertion. My assessment is that district nursing (extramural care) is needed for medication checks and help with ADLs. I propose that we schedule a first home visit within 24 hours (outreach). I would only recommend a temporary admission to a nursing home (intramural care) if his mobility or self-care does not improve or if no informal caregiver support is available.
Can you check whether the health insurer has any requirements for district nursing (e.g., contract or deductible), so that we can discuss this with the patient?
