Report – internal medicine ward (evening shift)

During the 20:00 round I measured the vital signs of the patient in room 12 and recorded them in the chart. The temperature was 38.4 °C; the patient reports feeling chilled. The heart rate was 108 beats per minute and irregular. The respiration was faster than earlier today; the oxygen saturation was 93% at rest. Blood pressure remained stable.

It was also noticeable that the skin is paler and the patient is less alert than this afternoon. The urine in the collection bag is darker and the volume is lower than in the previous round. I reported this immediately to the nurse on duty and asked for an extra check in one hour. Note: the patient is drinking little and coughing more.
