Elke maandag is er een op de afdeling geriatrie. De bespreekt eerst het van nieuwe patiënten. Daarna vertelt de hoe het met de dagelijkse zorg gaat. De geeft informatie over lopen en oefenen. De vertelt kort over het gedrag en de stemming van de patiënt. De kijkt naar problemen met geld of familie.
De maakt na de vergadering een kort verslag. Daarin schrijft zij wie welke taak krijgt. De let op de tijd en op de in het team. Aan het eind van de vergadering geven alle teamleden hun mening en kiezen samen de belangrijkste prioriteit voor die week.Every Monday there is a multidisciplinary team meeting on the geriatric ward. The doctor first discusses the care plan for new patients. Then the nurse reports on the daily care. The physiotherapist provides information about walking and exercises. The psychologist briefly describes the patient's behaviour and mood. The social worker looks at problems with money or family.
The case manager writes a short report after the meeting. In it she notes who is assigned which task. The team leader watches the time and the division of roles within the team. At the end of the meeting all team members share their opinions and together choose the main priority for that week.
