Intranetbericht: incident melden op de afdeling

There was an incident on the ward yesterday. A client became angry and showed aggression. Stay calm, keep your distance, and try to de-escalate. Do you feel unsafe? Then ask a colleague for help.

After each report, write a short incident report in the ECD. Write what happened and which conflict resolution you tried. In case of an attack or an allergic reaction: call the doctor immediately and get the AED if necessary. If there is sweating and trembling, also check for hypoglycemia.

Dear colleague,

Yesterday evening there was an incident involving Mrs Jansen (room 12). She suddenly became angry and shouted at the staff. A little later I saw that she became very pale and was trembling, possibly hypoglycemia. We spoke to her calmly to de-escalate and to monitor her safety. Afterwards a colleague measured her blood sugar.

Could you e-mail me briefly before 10:00 with what you saw and did? Then I will finish the incident report.

Beste Saskia,

Ik was aanwezig rond 20:15 in kamer 12. Ik zag dat mevrouw Jansen plotseling boos werd en hard riep. Ik bleef op korte afstand en sprak rustig met haar. Ik vroeg of ze wilde gaan zitten en of ze pijn of duizeligheid had. Voor de veiligheid heb ik collega Amir gevraagd om even te helpen.

Na enkele minuten werd mevrouw bleek en begon te trillen. Ik vermoedde hypoglykemie. Collega Amir heeft toen de bloedsuiker gemeten en mevrouw kreeg iets zoets te drinken. Daarna werd ze rustiger en kalmer.

Graag dat je in het incidentrapport noteert: verbale agressie, mogelijke hypoglykemie, onze de-escalatiestrategie (rustig praten, collega bijgeroepen) en de bloedsuikermeting met actie. Laat het me weten als je meer details nodig hebt.

Dear Saskia,

I was present around 20:15 in room 12. I saw that Mrs Jansen suddenly became angry and shouted loudly. I stayed at a short distance and spoke to her calmly. I asked if she wanted to sit down and whether she had pain or dizziness. For safety I asked colleague Amir to come and help for a moment.

After a few minutes Mrs became pale and started to tremble. I suspected hypoglycemia. Colleague Amir then measured her blood sugar and Mrs was given something sweet to drink. After that she became calmer and more settled.

Please note in the incident report: verbal aggression, possible hypoglycemia, our de-escalation strategy (speaking calmly, colleague called in) and the blood sugar measurement with action taken. Let me know if you need more details.
