Schmerz einschätzen auf der Inneren Station
Auf der Inneren Station des Städtischen Klinikums wird seit einem Jahr mit einer neuen Struktur für die Einschätzung von Patienten gearbeitet. Jede Pflegekraft dokumentiert bei Aufnahme zuerst die mit der numerischen von 0 bis 10. Zusätzlich beobachtet sie , Körperhaltung und , um nonverbale festzuhalten. Diese Informationen fließen in die und helfen, passende Interventionen zu planen. Bei uns wird auch das Modell der Gesundheit von Machteld Huber genutzt: Wir fragen nach Lebensqualität, Selbstmanagement-Fähigkeit und persönlichen Gesundheitsressourcen, nicht nur nach der Krankheit.
Für die Pflegeplanung orientiert sich das Team an den Gordon-Funktionsmustern. So werden Aktivitäts- und Ruheverhalten, Ernährungs- und Ausscheidungsverhalten sowie Bewältigungs- und Stressmuster systematisch erfasst. In der ärztlich-pflegerischen Übergabe nutzen wir die , um die Kommunikation zu strukturieren: Zuerst wird die Situation erklärt, dann die wichtigsten Beobachtungen beschrieben. Danach folgt die der und eine für das weitere Vorgehen. Am Ende holt die Pflegekraft aktiv Rückmeldung ein, damit alle Beteiligten die Prioritäten verstehen und die Patientenautonomie respektiert wird.On the Internal Medicine ward of the municipal hospital, a new structure for assessing patients has been in place for a year. On admission, each nurse first documents pain intensity using the numerical pain scale from 0 to 10. In addition, they observe facial expression, posture and protective movements to capture nonverbal signs of pain. This information is incorporated into the pain history and helps plan appropriate interventions. We also use Machteld Huber’s Positive Health model: we ask about quality of life, self-management abilities and personal health resources, not just the disease.
For care planning, the team follows Gordon’s functional health patterns. Activity and rest, nutrition and elimination, as well as coping and stress patterns are systematically recorded. In the medical–nursing handover we use the SBARR method to structure communication: first the situation is described, then the most important observations. This is followed by an assessment of urgency and a recommendation for further action. At the end the nurse actively requests feedback so that everyone involved understands the priorities and the patient’s autonomy is respected.

(In my assessment her pain intensity on the NRS is 7. The pain is in the right lower abdomen, has lasted about four hours, and is sharp. She also shows a clearly tense facial expression and a hunched posture.)
Pflegekraft Markus: Show Gut, das passt zu meiner Fremdbeurteilung, die Vitalzeichen waren aber noch stabil, oder?

Pflegekraft Anna: Show Genau, Vitalzeichen unauffällig, meine Empfehlung ist, dass der diensthabende Arzt die Medikation anordnet und wir bis dahin engmaschig überwachen, dokumentieren und die nonverbalen Schmerzanzeichen im Blick behalten.

Die Vitalzeichen sind leicht auffällig: Puls 102/min, Blutdruck 150/90 mmHg, Atemfrequenz 22/min, Temperatur 37,8 °C.
SBARR-Zusammenfassung: Frau König, 68 Jahre, starke Schmerzen im rechten Knie nach OP, trotz Analgetika keine ausreichende Wirkung (Situation). Postoperative Schmerzen, möglich unzureichende Schmerztherapie (Background/Assessment). Empfehlung: ärztliche Kontrolle der Medikation und Anpassung des Schmerzschemas.
Für die weitere Anamnese benötige ich noch genauere Informationen zu ihrer bisherigen Schmerztherapie zu Hause.

SBARR summary: Mrs. König, 68 years old, severe pain in the right knee after surgery; analgesics show insufficient effect (Situation). Postoperative pain, possibly inadequate pain management (Background/Assessment). Recommendation: medical review of medication and adjustment of the pain regimen.
For further medical history I still need more detailed information about her previous pain treatment at home.
