Außerdem beraten wir die Patientinnen und Patienten, ob ein Pflegeheim, Tagespflege oder ein ambulanter Pflegedienst sinnvoll ist oder ob die Versorgung allein zu Hause möglich bleibt.Our municipal social care center provides outpatient care and advice for older people in their homes. Many patients come to us after an inpatient hospital stay. We support follow-up care and coordinate home visits with the family doctor or a specialist. During the weekly consultation hour, relatives can ask questions about home care.
An important topic is long-term care insurance and health insurance. We help families clarify who will cover the costs and apply for the necessary benefits. If long-term care insurance denies a benefit, we inform them about deadlines and possible appeals. We also advise patients on whether a nursing home, day care, or an outpatient care service would be appropriate, or whether care at home can continue to meet their needs.
