Careful observation on the ward is important. During each round, check for any changes: skin color, amount of urine, consistency of stool or secretions, and whether the patient is unusually tired or restless. Also measure temperature, pulse, blood pressure and respiration. Pay attention to the level of consciousness: awake, drowsy or confused.

Record the values immediately in the nursing notes and briefly describe your observations. If values deviate from previous measurements, report directly to the responsible nurse or to the doctor—for example in case of fever, a very fast pulse or increased breathing. Also note when you measured and whether the patient has any complaints.
