Bei rheumatischen Erkrankungen sind möglich; dann braucht die Person oft mehr Unterstützung im Alltag.On the ward there are many patients with chronic conditions, for example diabetes, COPD, dementia or Parkinson’s. For nursing care it is important to know: What is the person’s condition today? If the person is stable, the usual measures are sufficient. If their condition worsens, the team needs increased monitoring and often an adjustment of treatment. Typical warning signs are extreme tiredness, worsening shortness of breath or new pain.
Also watch for heart failure. During a decompensation, problems can occur on the left or the right side. Left-sided: increased shortness of breath, rapid exhaustion, sometimes coughing. Right-sided: fluid buildup in the body, for example swollen legs (edema) and weight gain. Record symptoms in the medical history, review the medication and inform the physician in a timely manner. In rheumatic conditions flare-ups are possible; then the person often needs more support with daily activities.
