Instruction: Write a reply to the following message appropriate to the situation
E-Mail: You receive an email from the physiotherapist as a qualified nurse and should briefly reply in writing in German and provide information about the patient.
Betreff: Rehabilitation von Frau Novak – Bitte um kurze Rückmeldung
Liebe Frau / lieber Herr,
ich arbeite mit Frau Novak auf Station 3. Nach ihrem Schlaganfall möchten wir den Reha-Plan anpassen. Ich brauche bitte Ihre Einschätzung aus der Pflege.
Können Sie mir kurz schreiben:
- Wie ist die Mobilität von Frau Novak im Moment? Kann sie mit dem Rollstuhl oder mit Gehstock zur Toilette gehen?
- Wie klappt das Aufstehen, sich setzen und sich umdrehen im Bett? Braucht sie viel Hilfe beim Transfer vom Bett in den Stuhl?
- Hat sie aktuell Schmerzen? Wenn ja: Wie stark sind die Schmerzen auf der Schmerzskala von 0 bis 10?
Für das nächste Erholungsziel möchte ich gern planen, dass Frau Novak wieder sicher mit Hilfe zur Toilette gehen kann. Ihre Information hilft mir sehr für die nächste Verlaufskontrolle und die Evaluationssitzung morgen.

I am working with Mrs. Novak on Ward 3. After her stroke, we would like to adjust the rehab plan. I would appreciate your nursing assessment.
Could you briefly tell me:
- How is Mrs. Novak's mobility at the moment? Can she get to the toilet using a wheelchair or a walking stick?
- How are standing up, sitting down and turning over in bed going? Does she need a lot of help with the transfer from bed to chair?
- Does she currently have any pain? If so: How intense is the pain on the pain scale from 0 to 10?
For the next rehab goal, I would like to plan for Mrs. Novak to be able to go to the toilet safely with assistance. Your information will help me greatly for the next progress check and the evaluation session tomorrow.

Die Mobilität ist noch eingeschränkt. Sie fährt im Moment nur im Rollstuhl. Mit dem Gehstock kann sie noch nicht sicher gehen. Zur Toilette braucht sie immer eine Pflegekraft.
Beim Aufstehen braucht sie viel Hilfe. Sie kann sich im Bett langsam selbst umdrehen, aber sie ist noch unsicher. Beim Transfer vom Bett in den Stuhl muss ich sie stützen und absichern.
Frau Novak hat oft Schmerzen im linken Bein. Die Schmerzen sind meistens bei 4 bis 5 auf der Schmerzskala.
Für den Reha-Plan finde ich wichtig, dass wir weiter das sichere Aufstehen und Gehen üben.

Her mobility is still limited. At the moment she only moves around in a wheelchair. She cannot walk safely with a walking stick yet. She always needs a nurse to go to the toilet.
She requires a lot of assistance when standing up. She can slowly turn over in bed by herself, but she is still unsteady. During the transfer from bed to chair I need to support and secure her.
Mrs. Novak often has pain in her left leg. The pain is usually rated 4 to 5 on the pain scale.
For the rehab plan I think it is important that we continue practising safe standing up and walking.
